预后营养指数与脑梗塞患者预后的关系

摘　　要：目的探讨预后营养指数(prognostic nutritional index,PNI)与脑梗塞患者预后的关系。方法回顾性分析150例脑梗塞患者临床资料,搜集性别、年龄、白蛋白、淋巴细胞计数等基本资料,根据公式血清白蛋白值(g/L)+5×外周血淋巴细胞总数(×109/L)计算PNI进行统计分析。结果受试者工作特征曲线分析表明PNI预测脑梗塞预后的临界值为49.33。PNI在预后好与预后差的脑梗塞患者中的分布具有统计学意义(P<0.05),其余观察指标无显著性差异(P>0.05)。单因素分析表明中性粒细胞计数(OR:1.219;95%CI:1.007-1.476,P=0.043)、PNI(OR:0.247;95%CI:0.088-0.693,P=0.008)与脑梗塞预后相关,但纳入多因素分析后仅有PNI≥49.33(OR:0.261;95%CI:0.088-0.772,P=0.015)是脑梗塞患者预后的良好指标。结论 PNI≥49.33是脑梗塞预后的良好预测指标。Objective To study the association of prognostic nutritional index （PNI） with prognosis of patients with ischemic stroke. Methods The clinical data of 150 patients with ischemic stroke were retrospectively analyzed and the variables such as sex, age, albumin and lymphocyte count were collected. Statistical analysis was performed for PNI which calculated from the following formula： serum albumin （g/L） ＋ 5× peripheral lymphocyte count （ ×10^9/L）. Results The characteristic curve analysis about subjects indicated that cut-off of PNI in predicting the prognosis of patients with ischemic stroke was 49.33. There were significant difference of the distribution of PNI between patients with good prognosis and poor prognosis （P〈0.05）,the other variables did not have statistical significance （ P〉0.05 ）. Univariate analysis suggested that neutrophil count （ OR ： 1.219; 95% CI ： 1.007-1.476, P=-0.043 ）and PNI （ OR ： 0.247;95% CI： 0.088-0.693 ,P=0.008）were associated with prognosis of patients with ischemie stroke, while only PNI ≥49.33 （OR： 0.261 ;95%CI：0.088-0.772,P=0.015 ）was a favorable factor in predicting prognosis of patients harbored ischemie stroke when including them into multivariate analysis. Conclusion PNI≥49.33 was a favorable parameter in predicting clinical outcome of patients with ischemic stroke.
