三分支阻滞并位相阻滞伴隐匿传导致阵发高度房室阻滞1例

摘　　要：1病例资料患者,男,28岁,因劳累后出现头晕、黑朦,自觉心跳慢。既往无心肌炎及其他病史;家族中无50岁前猝死者。心脏超声为肥厚型非梗阻性心肌病（HCM）,以室间隔肥厚为主,最厚处为22 mm;左心房大;左室舒张功能减低。静息下12导联心电图特点（图1）：窦性P波顺序出现,PP间期0.80s,心率75次/min,PR间期0.24s,Ⅰ、avL导联呈rS型,Ⅲ、aVF导联呈qR型,A male patient of 28-year-old suffered from dizziness and amourosis after feeling fatigue.12 lead electrocardiogram showed that first degree atrioventricular block（I°AVB）,complete right bundle branch block（CRBBB）,left posterior fascicular block（LPFH）.24-hour ambulatory electrocardiogram showed that 3-phase block with concealed conduction to paroxysmal high degree atrioventricular block and 4-phase block to paroxysmal high degree atrioventricular block.
