美沙拉嗪肠溶片在激素抵抗型重症溃疡性结肠炎患者中的应用价值分析

摘　　要：目的研究美沙拉嗪肠溶片在激素抵抗型重症溃疡性结肠炎患者中的应用价值。方法选取宁夏回族自治区固原市人民医院消化内科2015年6月~2016年12月收治的110例激素抵抗型重症溃疡性结肠炎患者,随机均分为实验组与对照组（n=55）,对照组予以枯草杆菌二联活菌肠溶胶囊,实验组予以美沙拉嗪肠溶片,比较2组患者总体有效率、Mayo评分、细胞因子水平变化、C-反应蛋白与红细胞沉降率。结果实验组患者总体有效率为96.36%高于对照组85.45%（P〈0.05）。实验组患者Mayo评分为（3.17±2.03）分与对照组（3.69±1.97）分,差异无统计学意义。实验组白细胞介素-8、肿瘤坏死因子-α水平分别为（17.33±2.18）pg/mL、（18.33±2.23）ng/mL明显低于对照组（25.33±2.18）pg/mL、（25.93±1.54）ng/mL（P〈0.05）。实验组C-反应蛋白为（10.33±2.11）mg/L,ESR为（22.39±5.33）mm/h明显低于对照组（27.27±3.97）mg/L、（33.17±4.93）mm/h（P〈0.05）。结论美沙拉嗪治疗激素抵抗型重症溃疡性结肠炎的疗效确切,能够有效控制炎症,降低白细胞介素-8与肿瘤坏死因子-α水平,值得临床推广。Objective To study the application value of methadazine enteric-coated tablets in the treatment of steroid resistant severe ulcerative colitis. Methods 110 cases of patients with steroid resistance severe ulcerative colitis in our hospital from June 2015 to December 2016 were randomly divided into experimental group and control group（ n = 55）. The control group was given bacillus subtilis duplex living bacterium enteric capsules,the experimental group was given methadazine enteric-coated tablets. The total effective rate,Mayo score,cytokine levels change,C-reactive protein and erythrocyte sedimentation rate were Compared between two groups. Results The total effective rate of the experimental group was 96. 36% higher than that of the control group 85. 45%（ P 〈0. 05）. The Mayo score of the experimental group was（ 3. 17 ± 2. 03） and the control group was（ 3. 69 ± 1. 97）,the difference was not statistically significant. The levels of IL-8、TNF-α in the experimental group were（ 17. 33 ± 2. 18） pg/mL,（ 18. 33 ± 2. 23） ng/mL,which were significantly lower than the those of the control group（ 25. 33 ± 2. 18） pg/mL,（ 25. 93 ± 1. 54） ng/mL（ P 〈0. 05）. The C-reactive protein in the experimental group was（ 10. 33 ± 2. 11） mg/L the ESR（ 22. 39 ± 5. 33） mm/h,which was significantly lower than that of the control group（ 27. 27 ± 3. 97） mg/L,（ 33. 17 ± 4. 93） mm/h（ P 〈0. 05）. Conclusion Mesalazine is effective in the treatment of steroid resistant severe ulcerative colitis,which can effectively control the inflammation,,reduce the level of IL-8 and TNF-α,worthy of clinical promotion.
