Caspase-3在食管黏膜肌层平滑肌瘤中的表达及临床相关性

摘　　要：目的:探讨Caspase-3在食管黏膜肌层平滑肌瘤中的表达及临床相关性。方法:采用回顾性研究方法,选择我院消化内科及胸外科2011年1月到2016年8月经胃镜取材及手术切除食管标本90例,包括食管黏膜肌层平滑肌瘤30例(观察组)与正常食管上皮60例(对照组),两组均进行凋亡细胞检测与Caspase-3检测,并进行相关性分析。结果:Caspase-3在观察组与对照组标本中的的阳性表达率30.0%和71.7%,观察组明显低于对照组(P<0.05)。两组样本中均可检测到凋亡细胞,观察组与对照组的凋亡指数分别为(32.11±4.10)%和(22.10±4.95)%,观察组明显高于对照组(P<0.05)。在观察组中,Caspase-3的表达与肌瘤直径、临床分期有关(P<0.05),而与性别、年龄、肌瘤部位等无明显相关性(P>0.05)。结论:Caspase-3在食管黏膜肌层平滑肌瘤中呈现低表达状况,可导致细胞凋亡增加,且与临床分期、肌瘤大小呈明显相关性。Objective To investigate the expression and clinical relevance of Caspase-3 in esophageal mucosal leiomyoma.Methods A retrospective study of ninety esophageal specimens were selected from January 2011 to August 2016 at the Department of Thoracic Surgery and Department of Gastroenterology in our hospital including esophageal muscularis mucosa leiomyoma in 30 cases（ observation group） and normal esophageal epithelium in 60 cases（ control group）. Caspase-3 was detected in all specimens of the two groups and correlation analysis was performed accordingly.Results The positive expression rates of Caspase-3 in the observation group and the control group were 30.0% and 71.7% respectively,and the observation group was significantly lower than the control group（ P 0. 05）. The apoptotic cells could be detected in the two groups with the apoptotic index of（ 32.11＋4.10） % in the control group and（ 22.10＋4.95） % in the observation group. The observation group was significantly higher than the control group（ P 0. 05）. In the observation group,the expression of Caspase-3 was related to the diameter of the esophageal mucosal leiomyoma and clinical stage（ P0.05）,but there was no significant difference in gender,age,and the location of esophageal mucosal leiomyoma（ P0.05）. Conclusion Caspase-3 showed a low expression level in esophageal mucosa and myometrium,which could lead to an increase in apoptosis,and was significantly correlated with the clinical stage and the size of the esophageal mucosal leiomyoma.
