摘 要:目的探讨支气管动脉灌注治疗晚期非小细胞肺癌(NSCLC)的疗效及对血清肿瘤标志物含量、miRNA表达的影响。方法收集2012年5月至2015年12月间收治的晚期NSCLC患者89例,回顾治疗方案并将入组患者分为:接受常规静脉化疗的对照组患者45例、接受支气管动脉灌注联合常规化疗的观察组患者44例,连续化疗3个疗程。评估两组患者的近期疗效,采用酶联免疫吸附法(ELISA)检测血清肿瘤标志物含量、采用RT-PCR法检测肿瘤组织中miRNA表达量。结果化疗后,观察组的治疗总有效率高于对照组(P<0.05);观察组血清中肿瘤标志物细胞质胸苷激酶-1(TK-1)、非小细胞癌相关抗原(cyfra21-1)、中期因子(MK)含量均低于对照组(P<0.05);观察组肿瘤组织中miRNA-141、miRNA-221表达量低于对照组,miRNA-145表达量高于对照组(P<0.05)。结论支气管动脉灌注治疗晚期NSCLC可显著提升近期疗效,降低TK-1、cyfra21-1、MK含量,降低miRNA-141、miRNA-221表达量和提高miRNA-145表达量,从而降低肿瘤恶性程度。Objective To study the effect of bronchial artery infusion in Advanced non - small cell lung cancer patients and its influence on contents of tumor marker, miRNA expression. Methods 89 cases of Advanced NSCLC patients in our hospital during May 2012 to December 2015 were chosen as research subject, treatment scheme was reviewed and all patients were divided into control group (n = 45) who underwent routine intravenous chemotherapy, observation group (n = 44) who underwent bronchial artery infusion combined conventional chemotherapy, both lasted for 3 months. After chemotherapy, the short - term curative effect in two groups was evaluated, serum contents of tumor markers were meas-ured by ELISA. Expression of miRNA in tumor tissues was detected by RT - PCR assay. Results The total efficiency in observation group was higher than that in control group ( P 〈 0. 0 5 ) . Serum contents of tumor markers such as TK - 1 , CYFRA21 - 1 and MK in observation group were lower than those in control group ( P 〈 0. 0 5 ) . Expression of miRNA - 141 , miRNA -221 in tumor tissue in observation group were lower than those in control group, expression of miRNA - 145 was higher than that in control group ( P 〈0. 0 5) . Conclusion Bronchial artery infusion can significantly improve the short - term efficacy in advanced NSCLC patients, reduce the content of TK - 1 , cyfra21 - 1, MK, reduce the expression of miRNA - 141, miRNA - 221 and increase the expression of miRNA - 145 , reduce the degree of tumor malignancy.
