白细胞单采术治疗儿童高白细胞性急性白血病临床观察

摘　　要：目的探讨白细胞单采术在高白细胞性白血病（Hal）治疗中的意义。方法应用白细胞分离机对15例高细胞性白血病患儿进行37次白细胞单采术治疗，观察单采术前后血常规、生化及和凝血功能变化，以及不良反应和联合化疗的近期疗效。结果37次治疗中，27次显效。10次无效。白细胞单采术后白细胞、乳酸脱氢酶下降与单采治疗前比较有统计学差异（P〈0．05），但血红蛋白、血小板数量及血钾、血钙浓度，以及治疗前凝血功能正常患儿的凝血功能变化与单采治疗前相比无统计学差异（P〉0．05）；10例存活的急性淋巴细胞白血病（ALL）患儿经白细胞单采术联合化疗均达完全缓解。结论白细胞单采术能够快速降低Hal患者外周血白细胞计数，安全可靠，不良反应少，还可明显降低早期死亡率，提高缓解率，是一种有效的临床对症治疗手段。Objective To investigate the meaning of the leukapheresis in treatment of patients with high cellular leukemia（Hal）. Methods 15 patients with high cellular leukemia were cured with of leukapheresis for 37 times.To observe the change of hemogram, biochemistry and blood clotting function before and after the treatment,adverse effect and the newly therapeutic effect of combination chemotherapy.Result 10 times were ineffective and 27 times were excellence .The decrease of leucocyte and myocardial enzyme had statistical significance after the treatment of leukapheresis （Comaring with pre-treatment of leukapheresis, P 〈 0.05）.The count of hemoglobin and platelet,the concentrationg of serum potassium and blood calcium,the change of blood clotting function of the patients with normal blood clotting function before trement had not statistical significance （P 〉 0.05）.10 survival ALL patients were 100.00% complete remission after the treatment of leukapheresis combination chemotherapy（P 〈 0.05）. Conclusion Leukapheresis could quickly reduce the peripheral WBC counts of Hal patienta,Safe and reliable,Less adverse reaction,Still can significantly reduce the early mortality and improve the response rate,is a kind of effective means of clinical symptomatic treatment.
