摘　　要：目的:探讨乳清蛋白粉对重症急性胰腺炎(SAP)患者肠内营养的干预作用。方法:采用前瞻性研究方法,选择2015年1月-2016年12月我院重症医学科收治的50例SAP患者。随机将其分为对照组和观察组,对照组经鼻肠管泵入肠内营养混悬液,观察组经鼻肠管泵入肠内营养混悬液+乳清蛋白粉,记录患者治疗前及治疗7d后的前白蛋白(PA)、白蛋白(ALB)、APACHEⅡ评分,并比较两组患者治疗前、后各指标的差异,同时比较两组患者住院时间、住院费用及感染发生率的区别。结果:两组患者在治疗后对比于治疗前,PA、ALB均显著升高,APACHEⅡ评分显著下降(P<0.05)。治疗7d后,观察组PA、ALB明显高于对照组,APACHEⅡ评分明显低于对照组。观察组的住院时间、住院费用及感染发生率均明显低于对照组(P<0.05)。结论:乳清蛋白粉联合早期肠内营养可以明显改善重症急性胰腺炎的营养状况,可以改善预后,降低感染率,缩短住院时间及减少住院费用,并明显优于单纯肠内营养。Objective：To explore the Effect of whey protein powder on enteral nutrition in patients with severe acute pancreatitis（SAP）.Methods：A prospective analysis for 50 cases of severe acute pancreatitis was made for the study.All patients admitted to our ICU from Jannary 2015 to December 2016 were randomly divided into two groups：control group with 25 cases and observation group with 25 cases.The control group were treated with enteral nutritional suspension,while the observation group were treated with enteral nutritional suspension and whey protein powder.Enteral nutrition was used in all patients with nasal jejuna tube.Their pre-albumin（PA）,albumin（ALB）and APACHEⅡscore were recorded to compare.The differences of the length of hospital stay、hospitalization expenses and infection rate between the two groups were evaluated.Results：The PA and ALB levels of the two groups after treatment were higher than those before treatment,and APACHEⅡ score decreased significantly in the treatment.After 7 days of treatment,the PA and ALB in the observation group were significantly higher than those in the control group,and the score of APACHEⅡwas significantly lower than that of the control group.The length of hospital stay,hospitalization expenses and infection rate in the observation group were significantly lower than those in the control group.Conclusion：Whey protein powder combined with early enteral nutrition can significantly improve the nutritional status of the patients with severe acute pancreatitis,and significantly improve the prognosis,reduce the infection rate,shorten the hospitalization time and reduce hospitalization costs,and significantly better than simple enteral nutrition.
