恶性肿瘤患者静脉置管凝血参数与导管内血栓形成的关系分析

摘　　要：目的:通过监测恶性肿瘤行静脉置管患者血液中纤维蛋白原(Fibrinogen,Fib)、D-二聚体(D-dimer)及血小板(Platelet)计数,探讨其与导管内血栓形成的相关性。方法:采用免疫比浊法、阻抗法等对209例静脉置管的恶性肿瘤患者(分为血栓形成组、非血栓形成组)、79例未行静脉置管恶性肿瘤患者、30例健康对照者(健康对照组)进行Fib、D-dimer及PLT计数的检测。结果:恶性肿瘤置管血栓形成组、恶性肿瘤置管非血栓形成组、恶性肿瘤未行置管组Ddimer、Fib、PLT高于健康对照组。恶性肿瘤置管血栓形成组D-dimer明显高于其他组(P<0.05);TNM分期中Ⅲ、Ⅳ期恶性肿瘤置管组与Ⅰ、Ⅱ期组,差异有统计学意义(P<0.05)。恶性肿瘤置管非血栓形成组置管前、后相关参数异常率增加。结论:恶性肿瘤患者处于高凝状态,行静脉置管术时,血栓风险加大,与肿瘤分期关系密切;置管前相关参数异常者血栓发生率高,Fib、D-dimer及PLT可用于血栓早期风险提示。Objective：To detect correlation of blood fibrinogen（Fib）, D-dlmer anct platelet counter1, l J w,L. t in venous catheter in the malignant tumor patients. Methods.. A total of malignant tumor patients with venous catheter were enrolled, and 79 malignant tumor patients without venous catheter as well as 30 healthy volunteers were served as controls, while Fib,I）-dimer and platelet count were measured. Results,malignant tumor catheter thrombosis group, ma- lignant tumor catheter non-thrombosis group, the malignant tumor non-catheter group compared with healthy control group there were significant differences（P〈0. 05）Ⅲ、Ⅳmalignant tumor catheter groups compared with I , Ⅲ groups there were significant differences（P〈0.05）;Venous catheter jams in malignant tumor patients with catheter related parameters before and after the abnormal rate increases. Conclusion： The patients with malignant tumor when cathete- ring,thrombosis risk,and closely related to tumor stage; before the catheter related parameters abnormal increase the risk of blood clots, Fib, D-dimer and PLT can be used for early thrombosis risk warning.
