摘  要:为了探讨自然杀伤(natural killer,NK)细胞免疫球蛋白样受体基因KIRs与乳腺癌发生发展的关系,我们以45例乳腺癌患者为研究组,30例健康成年女性为对照组,分别采集其血液样本进行聚合酶链式反应(polymerase chain reaction,PCR),对两组中自然杀伤细胞免疫球蛋白样受体基因簇成员与其配体及其相关组合的表达频率进行了比较分析,发现与对照组相比,乳腺癌患者中KIR2DL5A、KIR2DS2和KIR2DS3 3种基因表达的频率显著降低,与乳癌症的发生发展成负相关关系,而KIR2DL1与KIR3DS1基因的频率稍高,但无统计学意义。基因的组合分析显示,KIR2DS2和KIR2DS3对乳腺癌患者具有一定的保护作用,在KIR2DS2缺失时KIR2DL2可成为危险因素。乳腺癌患者组内HLA-C1的基因型(93.3%)显著高于对照组(73.3%),抑制性的基因组合KIR2DL1+/HLA-C1C2+与乳腺癌之间存在正性相关关系;对于同一种KIR基因,负性相关关系见于KIR2DL1+/C2+(p<0.001)及KIR2DL1+/HLA-C2C2+(p=0.022)组合。我们的研究为了解乳腺癌的免疫遗传机制及其临床诊疗提供了一定的帮助。In order to explore the correlation between the natural killer cell immunoglobulin-like receptor gene, KIRs, and the occurrence and development of breast cancer, study group with 45 breast cancer patients and the control group with 30 healthy female adults was set for analysis, whose blood samples were collected and used for polymerase chain reaction. Then we compared and analyzed the expression frequency of the members of among natural killer cell immunoglobulin-like receptor gene clusters, their ligands and their combinations in two groups. The expression frequencies ofKIR2D5A, KIR2DS2 and KIR2DS3 in breast cancer were significantly lower than those in control group, which was negatively associated with the occurrence and development of breast cancer, while the expression frequencies of KIR2DL1 and KIR3DS1 in breast cancer samples were slightly higher, but the difference had no statistical significance. Gene combination analysis showed that KIR2DS2 and KIR2DS3 had a protective effect on breast cancer patients and that KIR2DL2 might be a risk factor in the absence ofKIR2DS2. The genotype ofHLA-C 1 (93.3%) in breast cancer patients was significantly higher than that in the control group (73.3%). The positive correlation between inhibitory gene combination of KIR2DL1 +/HLA-C1C2 + and breast cancer was also found. For the same KIR gene negative correlation was found in the combination of KIR2DL 1 +/C2+(p 〈0.001) andKIR2DL1 +/HLA-C2C2+(p =0.022). Our research provided some help for the understanding the immunogenetics mechanism of breast cancer and its clinical treatment.
