摘  要:目的:观察肿瘤患者经微量泵注入替加氟与静脉滴注所产生的不良反应.方法:选取2014-06/2016-12经汕头大学医学院附属肿瘤医院病理确诊为头颈癌的46例患者和直肠癌的23例患者作为研究对象,随机分为微量泵入组(n=38)和静脉滴注组(n=31).比较两组间不良反应发生率和严重程度.结果:两组每位患者均完成3~8个周期,微量泵入组共完成233个周期,静脉滴注组共完成189个周期.两组患者根据周期统计不良反应总发生率微量泵入组及静脉滴注组分别为81.5%和79.9%,差异无统计学意义(P>0.05);微量泵入组及静脉滴注组血液系统、白细胞减少、中性粒细胞减少、贫血、血小板减少的不良反应发生率分别为:36.5%<55.0%、36.1%<50.3%、33.5%<48.7%、31.3%<41.3%及18.0%<33.3%,差异有统计学意义(P<0.05).微量泵入组及静脉滴注组非血液系统中口腔黏膜损害、周围静脉炎、手足综合征及指甲病变的不良反应发生率分别为:15.0%<41.8%、3.4%<16.4%、6.9%<50.6%及4.8%<36.5%,差异有统计学意义(P<0.05),两组非血液系统其他胃肠道方面的不良反应发生率比较,差异无统计学意义(P>0.05).结论:替加氟微量泵入能减轻腹泻、骨髓抑制及口腔黏膜炎的发生,不良反应较静脉滴注组轻.AIM: To observe the adverse reactions of tegafur given by micro-pump and intravenous drip in tumor patients. METHODS: A total of 46 cases diagnosed with head and neck cancer and 23 cases diagnosed with rectal cancer were randomly divided into intravenous drip group and micro-pump group. The former included 31 cases and the latter included 38 cases. The difference of incidence of adverse reactions and its severity be- tween the two groups were compared. RESULTS: Each patient was given 3 to 8 courses of treatment. Intravenous drip group com- pleted 189 ~oles, attd micro-pump group completed 233 cycles. There was no significant difference in the total incidence of adveme reaction between the two groups (81.5% vs 79.9%, P〉 0.05). However, the incidences of adverse reaction of micro- pump group in blood system, leukopenia, neutropenia, anemia, thrombocyte were significantly lower than the intravenous drip group, which were respectively 36.5%〈55.0%, 36.1%〈50.3%, 33.5%〈48.7%, 31.3%〈41..3% and 18.0%〈33.3%, and there were statistically significant differences between the two groups (P〈0.05). In addition, the incidences of oral mucosa damage, peripheral phlebitis, hand-foot syndrome and nail damage in mi- cro-pump group were significantly lower than the intravenous drip group, which were respectively 15.0% 〈 41.8%, 3.4% 〈 16.4%, 6.9%〈50.6% and 4.8%〈36.5%, and there were statistically sig- nificant differences between the two groups ( P 〈 0.05 ). While there was no statistically significant difference in the incidence of non-blood system and other gastrointestinal system ( P 〉 0.05 ).CONCLUSION: The administration of tegafur given by micro- pump could relieve diarrhea, bone marrow suppression and inflammation of oral mucosa with milder adverse reactions than intravenous drip.
