神经生长因子凝胶保护青光眼视神经的临床效果

摘　　要：目的探讨神经生长因子(NGF)凝胶对原发性闭角型青光眼患者术后视神经的保护作用。方法选取60例86眼原发性青光眼患者为研究对象,随机分为观察组(小梁切除术后采用NGF凝胶做后续治疗)28例42眼和对照组(术后采用三磷酸腺苷二钠片作后续治疗)32例44眼,所有患者在结束手术后坚持用药4 w,比较两组患者术后用药前和术后2、6个月的视力、眼压、视野、视网膜神经纤维层厚度(RNFL)和视乳头杯盘比。结果观察组患者术后用药前视力为0.41±0.17,术后2个月为0.50±0.20,术后6个月为0.49±0.25,对照组患者术后用药前视力为0.42±0.22,术后2个月为0.42±0.19,术后6个月为0.41±0.28,观察组术后2、6个月的视力均高于同期的对照组(P均<0.05)。两组患者眼压在术后用药前、术后2、6个月均没有统计学差异。观察组视野平均视敏度在术后2、6个月均明显高于对照组,视野平均缺损值在术后6个月低于对照组(P<0.05)。观察组患者在术后6个月时,RNFL明显大于对照组,杯盘比明显小于对照组(P<0.05);但在术后2个月两组患者无统计学差异。结论NGF凝胶用于原发性青光眼患者术后治疗有显著的临床效果,对视神经有很好的保护作用。Objective To investigate the protective effects of nerve growth factor （NGF） gel on the optic nerve after primary angleclosure glaucoma patients. Methods 60 cases of 86 primary glaucoma patients were randomly divided into two groups. After 2, 6 months, visual acuity, intraocular pressure, vision, retinal nerve fiber layer thickness （RNFL） and optic papilla cup-disc ratio test results were ana- lyzed and compared. Results The visual acuity of patients in observation group was 0.41 ±0. 17, after two months 0.50 ±0.20, after 6 months 0.49±0.25; the visual acuity of the patients in control group was 0.42±0. 22, after 2 months 0.42±0. 19, after 6 months 0. 41±0. 28. After 2 and 6 months, the visual acuity of observation group were higher than those of control group （ P〈0.05 ）. IOP data had no significant differences between the two groups; and the average visual acuity after 2 and 6 months in observation group were significantly higher than those of control group, the mean visual field defect values after 6 months was lower than that of control group. The retinal nerve fiber layer thickness in observation group after 6 months was significantly greater than that of control group, cup-disc ratio was significantly lower than that of control group. Conclusions The NGF gel could protect the optic nerve of Glaucoma.
