血清外泌体miR-21在肺癌中的表达水平及其诊断效能

摘　　要：目的探索外泌体miR-21在肺癌患者血清中的表达水平及诊断效能。方法收集南方医院肺癌患者94例,肺结核患者29例,健康对照26例。纯化鉴定血清外泌体,提取miRNA,进行RT-PCR,比较各组间miR-21表达水平。利用ROC曲线评价外泌体miR-21诊断效能。结果纯化样品具备已有报道的外泌体的典型特征。外泌体miR-21表达水平在肺癌、肺结核、健康对照3组间存在显著性差异;外泌体miR-21在肺癌组中显著升高,肺结核组显著降低。肺癌组中,早、晚期肺癌患者外泌体miR-21与健康对照间均存在显著性差异。外泌体miR-21的ROC曲线的曲线下面积为0.702,且具有较好的特异性。结论血清外泌体miR-21具有较高的诊断效能,有望作为诊断标志物,辅助肺癌临床诊断。Objective To observe the expression level and diagnosis performance of serum exosomal miR-21 in lung cancer.Method Sera of 94 lung cancer patients,29 pulmonary tuberculosis patients,and 26 healthy controls from Nan Fang Hospital were collected.Exosomes in serum were purified and characterized,and miRNA was abstracted from exosomes.Levels of exosomal miR-21 in three groups were compared with relative quantitative RT-PCR.Diagnosis performance of exosomal miR-21 was evaluated by ROC curve.Results Purified serum exosome sample possesses typical exosome features.There were significant difference among exosomal miR-21 levels from lung cancer,tuberculosis,and health control group.Exosomal miR-21 obviously increased in lung cancer patients,and decreased in tuberculosis patients.Exosomal miR-21 were increased in early and late stage lung cancer patients.The diagnostic performance of exosomal miR-21 was evaluated by ROC curve.Its area under the curve was0.702 and specificity was 0.923.Conclusions Serum exosomal miR-21 was increased in many kinds of lung cancer patients.It possesses excellent diagnosis performance,which may be developed as new biomarkers to assist lung diagnosis.
