miRNA-21与高血压并腔隙性梗死的相关性及预测价值

摘　　要：目的探讨微小RNA-21(miR-21)与高血压伴腔隙性梗死(腔梗)的相关性及诊断预测作用。方法将67例高血压患者根据是否合并腔梗分为高血压组31例和高血压并腔梗组36例,收集各项临床和实验室指标,采用实时荧光定量PCR(qRT-PCR)测量血清miR-21;绘制受试者工作曲线(ROC),评价miR-21对高血压并腔梗的诊断预测价值,通过Pearson相关分析了解miR-21与颈动脉IMT的相关性。结果高血压并腔梗组miR-21表达(2.08±0.61),显著高于高血压组(1.25±0.37,P<0.05)。miR-21对高血压发生腔梗的ROC曲线下面积(AUC)为0.871,95%可信区间(CI)0.787~0.954(P<0.05)。Pearson相关分析发现miR-21与颈动脉IMT呈正相关(R=0.763,P<0.05)。结论 miR-21在高血压并腔隙性脑梗死患者中高表达,是高血压发生腔梗的独立预测因素,可作为高血压发生腔梗的一个简便、无创、有效的血清学诊断预测指标。Objective To investigate the expression and diagnosis of miRNA-21 for being used as a prediction target molecular marker of lacunar infarction with hypertension.Methods 67 cases of patients with hypertension was divided into hypertension group（31 cases）and hypertension with lacunar infarction group（36 cases）.Then measured serum miR-21 using real-time fluorescent quantitative PCR（qRT-PCR）and collected the clinical and laboratory indexes;evaluated miR-21 predictive value to the diagnosis of hypertension with lacunar infarction through receiver operator characteristic curve（ROC）.Then studied the pertinence of miR-21 and the carotid artery intima-media thickness（IMT）using Pearson correlation analysis.Results The expression of miR-21 in serum of hypertension with lacunar infarction patients was significantly higher than that of the hypertension group（2.08±0.61 VS.1.25±0.37,P〈0.05.ROC analysis results showed that the area under the ROC（AUC）was 0.871,and 95% confidence interval（95% CI）was 0.787~0.954.Pearson correlation analysis found that miR-21 and carotid IMT were positively correlated（R=0.763,P〈0.05）.Conclusion The serum level of miR-21 in hypertension with lacunar infarction was significantly higher than that in hypertension patients.The miR-21 has certain predictive value of lacuncar infarction in hypertension.
