摘　　要：目的通过Meta分析评价尤瑞克林联合氯吡格雷治疗进展性脑梗死的有效性及安全性。方法通过计算机检索相关数据库,由两名研究者独立进行文献筛选和数据采集,使用Rev Man5.3软件进行Meta分析。结果共纳入7项随机对照试验研究,样本数为517例,其中治疗组(尤瑞克林联合氯吡格雷)252例,对照组265例。Meta分析结果显示尤瑞克林联合氯吡格雷组对急性进展性脑梗死的疗效较对照组显著提高,差异具有统计学意义(P<0.05);同时改善神经功能缺损程度显著优于对照组,差异有统计学意义(P<0.05)。对其中3项研究恶化率的Meta分析结果显示,尤瑞克林联合氯吡格雷组较对照组明显降低,差异有统计学意义(P<0.05)。结论尤瑞克林联合氯吡格雷能有效地改善急性进展性脑梗死的神经功能缺损症状,且无明显严重不良反应。受纳入研究质量所限,上述结论仍需更多大样本、多中心、前瞻性的随机对照研究加以验证。Objective To assess the effects and safety of Urinary Kallidinogenase combined with clopidogrel for patiens with progressive cerebral infarction using Meta-analysis.Methods We retrieved relevant databases through computer and two independent researchers conducted literature screening and data collection.We used RevMan5.3 software for Meta-analysis.Results A total of 7 randomized controlled trials were included in this Meta-analysis.The sample number was 517 cases with including 252 cases in treatment group（Urinary Kallidinogenase combined with clopidogrel）and 265 cases in control group.Meta-analysis results showed that the effect of Urinary Kallidinogenase combined clopidogrel group on acute progressive cerebral infarction was significantly higher than control group,and the difference was statistically significant（P〈 0.05）;The degree of Urinary Kallidinogenase combined with clopidogrel group improving neural function defect was better than control group,and the difference was statistically significant （P〈 0.05）.The deterioration rate Meta-analysis result of 3 study showed that Urinary Kallidinogenase combined with clopidogrel group was significantly lower than control group,and the difference was statistically significant （P〈 0.05）.Conclusion Acute progressive cerebral infarction treatment of Urinary Kallidinogenase combined with clopidogrel could effectively improve nerve system function defect degree and have no obvious adverse reactions.By the limitation of study quality,the above conclusions still needed to be verified by much more large sample,multi-center,and prospective randomized controlled study.
