缝扎注射平阳霉素碘化油乳剂治疗颌面部静脉畸形的研究

摘　　要：目的评价平阳霉素碘化油乳剂(pingyangmycin oleum iodisatum emulsion,PLE)的药物效价;分析缝扎注射PLE治疗口腔颌面部静脉畸形的临床疗效和不良反应。方法采用碘化油与平阳霉素(pingyangmycin,PYM)混合后碘含量测定,评价PYM的药物效价;通过X线透视观察缝扎注射后病变内碘化油滞留情况;记录、分析治疗病例的临床疗效及不良反应。结果 PYM与碘化油混合后碘含量不变;X透视显示,4h内病变碘化油浓度无明显改变,4~30h逐渐排空;40例患者经缝扎注射PLE均治愈,随访12~24个月无复发;全部病例均未出现肺纤维化、脑栓塞、肺栓塞、呼吸困难、组织坏死及功能障碍等症状。结论 PYM与碘化油混合后药物效价不受影响;缝扎注射PLE治疗口腔颌面部静脉畸形,可有效增加治疗药物对静脉畸形的直接作用时间和浓度,显著提高临床治疗效果,减少不良反应。Objective To investigate the potency and efficiency of Pingyangmycin Oleum Iodisatum Emulsion （PLE）, and analyze the clinical efficacy of ligation and injection of PLE for treating venous malformations in maxillofacial region and relevant adverse reactions. Methods The potency and efficiency of Pingyangmycin （PYM） were evaluated with detection of iodine content after mixing oleum iodisatum with PYM. X-ray fluoroscopy was used to observe the retained condition of oleum iodisatum in the lesion after ligation and injection, to record and analyze the clinical efficacy and adverse reactions. Results The iodine content was without change in mixture of oleum iodisatum and PYM; X-ray fluoroscopy showed that concentration of oleum iodisatum was without obvious change in lesion within four hours after ligation and injection, but gradually drained out from four hours to 30 hours. A total of 40 cases were cured after ligation and injection of PLE, and no recurrence was found with follow-up for 12 to 24 months. All cases were without pulmonary fibrosis, cerebral embolism, pulmonary embolism, difficulty in breathing, tissue necrosis, and dysfunction or other symptoms. Conclusion The potency and efficiency of PYM is without change after mixing with oleum iodisatum. Ligation and injection of PLE for treating venous malformations in maxillofacial region can effectively increase the direct action time and concentration of therapeutic drug on venous malformations, significantly improve clinical efficacy, and reduce adverse reaction.
