腹腔镜前列腺癌根治术的初步体会

摘　　要：目的:总结腹腔镜前列腺癌根治术围手术期并发症发生情况,并分析原因,总结手术经验体会。方法:回顾分析2011年1月至2016年3月开展的55例腹腔镜前列腺癌根治术患者的临床资料及术后随访资料,评估围手术期并发症情况。结果:55例患者均成功完成腹腔镜手术,无一例中转开放手术。术后平均住院(16.69±2.92)d,术后留置尿管时间平均(14.73±1.41)d,未发生多器官功能衰竭或死亡病例,并发症严重程度评估依据Clavien系统分级,共发生2例轻微并发症,为吻合口瘘、尿失禁各1例,并发症发生率为3.6%,未发生其他严重并发症。结论:腹腔镜前列腺癌根治术在围手术期并发症控制方面具有一定优势,但腹腔镜前列腺癌根治术的学习曲线较长,需要一定的腹腔镜手术经验及手术技巧的积累。Objective：To summarize the perioperative complications and operation experience of laparoscopic radical prostatectomy（LRP）,and analyze the causes of complications.Methods：The case records of 55 patients who underwent LRP for prostate cancer from Jan.2011 to Mar.2016 were retrospectively reviewed,including clinical data during hospitalization and follow-up after surgery in order to assess the perioperative complications.Results：All laparoscopic operations of 55 patients were successful,no conversion to open surgery was necessary in any cases.The mean postoperative hospital stay was（16.69 ± 2.92） d.The mean duration of bladder catheterization was（14.73 ± 1.41） d.There was no multiple-organ dysfunction or death.The severity of complications were recorded according to the Clavien grading system.2 minor complications occurred and were appropriately treated,one was anastomotic fistula and the other was urinary incontinence,with the incidence of 3.6%.No severe complications were observed.Conclusions：LRP has advantages in controlling complications during the perioperative period.But there is still a longer learning curve,therefore,the surgeons need more experience accumulation of laparoscopic surgeries.
