肝细胞癌患者外周血Th22细胞检测及临床意义

摘　　要：目的探讨肝细胞癌患者外周血Th22细胞表达水平及其临床意义。方法抽取52例肝细胞癌患者、38例慢性乙型肝炎患者、32例健康对照者外周血标本,采用流式细胞术检测Th22细胞水平,ELISA法检测IL-22水平,分析肝细胞癌组Th22细胞、IL-22与肿瘤标志物AFP、CEA、CA199的相关性。结果外周血Th22细胞和IL-22水平在肝细胞癌组、慢性乙型肝炎组显著高于健康对照组(P<0.05),肝细胞癌组略高于慢性乙型肝炎组(P>0.05);肝细胞癌患者外周血Th22细胞和IL-22水平存在正相关(r=0.342,P<0.05),而外周血Th22细胞、血浆IL-22水平与肿瘤标志物AFP、CEA、CA199均无相关性(P>0.05)。肝细胞癌组HBV阴、阳性患者间Th22细胞和IL-22水平比较均具有统计学差异(P<0.05)。结论 Th22细胞在肝细胞癌外周血中呈高水平表达,且与乙型肝炎病毒携带可能相关,但与血清肿瘤标志物无相关性,提示该细胞可能参与了肝细胞癌的发生和进展。Objective To investigate the expression of Th22 cells in peripheral blood of hepatocellular carcinoma patients and its clinical significance. Methods The peripheral blood were collected from 38 cases with chronic hepatitis B and 32 healthy controis. The expression level of Th22 ceils in blood samples was detected by flow cytometry （FCM）;the IL-22 was detected by an enzyme-linked immunosorbent assay （ELISA）. The potential interrelationship of Th22 cells,IL-22 in peripheral blood and tumor markers including AFP,CEA and CA199 was performed. Results The expression level of Th22 cells and IL-22 in peripheral blood of hepatocellular carcinoma group and chronic hepatitis B group were significantly higher than those of healthy control group （P〈0.05）, which of the hepatocellular carcinoma group were slightly higher than those of the chronic hepatitis B group（P〉0.05）. The percentage of Th22 cells in hepatocellular carcinoma group had the positive correlation with the level of IL-22 （r=0.342,P〈0.05）, while the expression level of Th22 cells and IL-22 had no correlation with tumor markers inch/ding AFP, CEA and CA199 （P〉0. 05）. Additionally,the expression level of Th22 cells and IL-22 bore significant difference in the HBV-negative and -positive hep- atocellular carcinoma patients （P〈0.05）. Conclusion Th22 cells in the peripheral blood of hepatocellalar carcinoma patients showed high expression level,with which hepatitis B virus carrying might be related, but not serum tumor markers. The results suggested that the Th22 cells might participate in pathogenesis and progression of hepatocellular carcinoma.
