达芬奇机器人手术系统辅助游离腓骨瓣修复下颌骨及口底缺损一例报告并文献复习

摘　　要：目的探讨达芬奇机器人手术系统辅助游离腓骨瓣修复恶性肿瘤切除后下颌骨及口底缺损的临床疗效。方法对1例下牙龈鳞状细胞癌的患者进行达芬奇机器人手术系统辅助游离腓骨瓣修复下颌骨及口底缺损的手术，手术分两组同时进行：一组行原发灶切除；另一组行右小腿游离腓骨瓣切取。利用达芬奇机器人手术系统进行血管的吻合及口底黏膜的缝合。结果术后4周随访．皮瓣完全成活，口底无感染．可自由行走和进食半流质。结论达芬奇机器人手术系统相较于传统的显微外科手术具有更加的精细稳定、创伤更小、视野更清楚、恢复更快的优势，将是复杂显微外科今后新的发展方向。Objective To discuss the clinical effect of Da Vinci robot-assisted free fibula flap for the management of mandibular and mouth segmental defect. Methods There was a patient with malignant neoplasm of lower gum, which underwent robot-assisted free fibula flap for the management of mandibular and mouth segmental defect, was analyzed. The surgery was divided into two groups： a group resected the primary tumors, another group cut fibula free flap. Use of Da Vinci robotic surgical system for vascular anastomosis and suture of the oral mucosa. Results Four weeks after surgery, the free fibula flap survived completely. There was no infection in the mouth. The patient can walk freely and eating semifluid, Conclusion The Da Vinci robotic surgical system is a stable, minimally invasive, clear and safe method for microsurgery.
