白内障超声乳化摘除+人工晶体植入术+小梁切除术治疗青光眼合并白内障临床疗效

摘　　要：目的比较青光眼合并白内障采用不同手术方式治疗的临床效果。方法选择2014年9月至2016年9月我院收治的84例青光眼合并白内障患者,随机分为观察组和对照组,每组42例。对照组患者采用人工晶体植入术联合白内障囊外摘除术,观察组患者则采用白内障超声乳化摘除+人工晶体植入术+小梁切除术,对比两组患者治疗前、后的眼压情况、视力情况以及相关并发症发生率。结果治疗后,两组患者的眼压均下降(P<0.05),但组间差异不显著(P>0.05)。观察组患者视力≥5.0、4.7~4.9、4.5~4.6占比明显高于对照组,4.0~4.4、<4.0占比明显低于对照组(P<0.05)。对照组并发症总发生率为11.9%(5/42),观察组则为9.52%(4/42),组间差异不显著(P>0.05)。结论在临床治疗青光眼合并白内障患者过程中,可以采用白内障超声乳化以及人工晶体植入,同时联合小梁切除术疗效显著,且不会提高并发症发生率。Objective To compare the clinical effects of different surgical methods in the treatment of glaucoma complicated with cataract. Methods Eighty-four cases of patients with glaucoma and cataract admitted in our hospital from September 2014 to September 2016 were selected and randomly divided into observation group and control group. The control group was given combination of intraocular lens implantation and extracapsular cataract extraction, while the observation group accepted phacoemulsification＋intraocular before and after treatment, visual acuity and the incidence lens implantation ＋trabeculectomy. The intraocular pressure rate of related complications between the two groups were compared. Results After treatment, the intraocular pressure of both groups decreased （P〈0.05）, but there was no significant difference between the two groups （P〉0.05）; the proportions of visual acuity ≥5.0, 4.7-4.9 and 4.5-4.6 in the observation group were significantly higher than those in the control group and 4.0-4.4, 〈4.0 were lower than the control group （P〈0.05）. The total incidence rate of complications in the observation group was 11.9% （5/42） and it was 9.52% （4/42） in the observation group, the difference was not statistically significant between the two groups （P〉0.05）. Conclusion In the clinical treatment process of patients with glaucoma and cataract, the combination of phacoemulsification ＆ intraocular lens implantation and trabeculectomy has remarkable effect, and it doesn＇t increase incidence of complications.
