微球法检测干眼患者泪液中IFN-γ与IL-17细胞因子浓度的相关分析

摘  要:目的通过微球法(Cytometric bead array,CBA),即微量样本多重蛋白定量技术测定干眼患者与正常对照组泪液中的两种细胞因子IFN-γ(Interferon-γ)与IL-17(Interleukin-17)的浓度,探讨炎症因素在干眼的发病机制中的作用。分析各组泪液中IFN-γ与IL-17两种细胞因子的浓度与临床检查结果的相关性。方法本研究为2015年至2016年内蒙古医科大学附属医院眼科门诊就诊的患者,进行包括视力、泪膜破裂时间(Break up time,BUT)、泪液分泌实验I(Schirmer I test,SⅠt),依据日本干眼标准(2005)筛选功能性干眼患者21例(21眼:14例右眼,7例左眼),其中男4人(4眼:2例左眼,2例右眼),女17人(17眼:12例右眼,5例左眼),眼别筛选依据患者自觉症状较为严重的一侧。依据正常对照组标准筛选健康志愿者18例(18眼:11例右眼,7例左眼),其中男4人(4眼:2例左眼,2例右眼),女14人(14眼:9例右眼,5例左眼),眼别选取与病例组性别相匹配(两组间每侧眼男女比例接近1:1)。使用毛细管法收集上述两组泪液。通过CBA法检测泪液样品中IFN-γ与IL-17的浓度,并与临床检查结果进行相关分析。结果正常组与对照组的年龄、性别无统计学差异(P=0.558)。两组被检者SⅠt有统计学差异(P<0.001);两组泪液样本中IL-17浓度与SⅠt有统计学意义(r=0.38,P=0.001);被检者SⅠt结果与BUT结果相关性有统计学意义(P<0.001)。结论 (1)两组被检者泪液中的细胞因子浓度并未发现有统计学差异,但病例组IFN-γ浓度中位数较对照组高。(2)两组被检者SⅠt结果与BUT结果有统计学差异,说明传统方法检测仍是不可代替的检查方法。且SⅠt与BUT是评估干眼的敏感指标。(3)被检者IL-17浓度与SⅠt结果存在正相关性,推断IL-17浓度的增加对泪液的减少分泌是一种保护性因素。Objective The concentrations of two cytokines IFN-γ(Interferon-γ)and IL-17(INTERLEUKIN-17)in tear in dry eyes and normal control group were measured by Microsphere method(Cytometric Bead Array, CBA,which is a micro-sample multi- ple protein quantification technique), and the role of inflammatory factors in the pathogenesis of dry eye was investigated.2.Analysis of the correlation between the concentrations of IFN-γ and IL-17 two cytokines in tear and the Clinical examination results: Methods For the 2015-2016, Inner Mongolia Medical University affiliated hospital ophthalmic outpatient clinic patients, including vision, tear film rupture time (break up Time, BUT), tear secretion experiment I (Schirmer i test, S I t), According to Japan Dry Eye Standard (2005)screening functional Dry eye patients 21 cases (21 eyes;14 cases of the fight eye,7 cases left eye, the male 4 (4 eyes,2 case left eye,2 case fight eye)female 17 (17 eyes;12 cases fight eye,5 case left eye),eye screening according to patient conscious symptom more serious side.Screening healthy volunteers in 18 cases (18 eyes, 11 right eyes,7 left eyes)According to the normal control group.Among the men 4(4 eyes,2 cases left eye,2 case fight eye)female 14( 14 eyes,9 cases of fight eye,5 case left eye),eye selection and case Group Sex match (two group between men and women in each side of the proportion of close to 1:1 ). The two groups of objects were collected by Capillary method.The concentration of IFN-~/ and IL-17 in tear samples was detected by CBA method,and the correlation was correlated with clinical examination resuhs.Results There was no statistically difference between the age and sex of the normal group and the control group(P=0.558).There were statistically significant differences between the two groups of S I t (P〈0.001);the correlation between IL-17 concentrations and S-t resuhs in two groups of tear samples was statistically significant (r=0.38,P=0.001);the correlation of S
