补肾解郁清心汤治疗更年期抑郁症疗效及对DA、NE、5-HIAA的影响

摘　　要：目的观察补肾解郁清心汤治疗更年期抑郁症患者的临床疗效及对多巴胺(DA)、去甲肾上腺素(NE)、5-羟吲哚乙酸(5-HIAA)的影响。方法将76例更年期抑郁症患者随机分为观察组和对照组。2组均给予常规治疗,观察组在此基础上给予自拟补肾解郁清心汤治疗,以2周为1个疗程,治疗4个疗程。观察2组临床症状、抑郁及焦虑程度改善情况,检测治疗前后血清神经内分泌激素及生殖内分泌激素水平。结果观察组治疗后总有效率明显高于对照组(P<0.05);治疗后,2组HAMD、HAMA及MI评分均显著降低(P均<0.05),且观察组治疗后评分均显著低于对照组(P均<0.05);治疗后,2组血清DA、NE、5-HIAA水平均显著升高(P均<0.05),且观察组上述指标水平均显著高于对照组(P均<0.05);治疗后,2组血清E2含量均显著升高(P均<0.05),FSH和LH含量均显著降低(P均<0.05),且观察组上述各指标改善情况均显著优于对照组(P均<0.05)。结论补肾解郁清心汤通过改善更年期抑郁症患者下丘脑-垂体-性腺轴和甲状腺轴功能状态,提高体内单胺类物质的含量,具有明显的抗抑郁作用。Objective It is to observe the effect of Bushen Jieyu Qingxin decoction on climacteric depression effect and influence on DA, NE, 5-HIAA. Methods 76 patients with climacteric depression were randomly divided into observation group and control group. Both groups were given conventional treatment, additionally, the observation group was treated with Bushen Jieyu Qingxin decoction. 2 weeks as one treatment course, both groups were treated for 4 courses. The improvements of clinical symptoms, depression and anxiety were observed and the levels of serum neural endocrine hormone and reproductive endo-crine hormone before and after treatment were detected in both groups. Results The total effective rate in observation group is significantly higher than that in control group after treatment （ F 〈0. 05） . After treatment, the scores of HAMD, HAMA and MI were significantly decreased（F 〈0.05） , and the scores were significantly lower in observation group than that in control group （ F 〈 0. 05 ） ;The levels of serum DA, NE , 5-HIAA, E2 were significantly increased （ F 〈 0.05 ） , while that of FSH and LH were significantly decreased（ F 〈 0. 05 ） , and the improvements of these indexes in observation group were significantly better than that of control group （F〈0.05）. Conclusion Bushen Jieyu Qingxin decoction has obvious antidepressant effect by improving menopausal depression the hypothalamus-pituitary gonad axis and thyroid axis function, raising the content of single amine substance in the body.
