角膜波前像差引导的准分子激光原位磨镶术和准分子激光角膜上皮瓣下磨镶术对高阶像差的影响

摘　　要：目的:研究分析波阵面像差引导的准分子激光原位角膜磨镶术(W-G-LASIK)和准分子激光角膜上皮瓣下磨镶术(W-G-LASEK)针对近视眼高阶像差产生的影响,并作对比观察分析。方法:角膜波前像差引导治疗近视61例(122眼),其中W-G-LASIK手术(A组)共27例(54眼)和W-G-LASEK手术(B组)共34例(68眼),分别于术前和术后7 d,1个月,3个月测量高阶像差(HOAs),并分析手术前后HOAs的变化和两种手术方式对HOAs产生的影响。结果:两组术后总体HOAs均方根值均较术前有所增加(P<0.05),以三、四阶像差增加为主,五、六阶像差则较术前变化不明显,术后B组的总HOAs、三阶、四阶像差均小于A组(P<0.05)。两组患者的慧差和球差均比术前有所增加(P<0.05),其中A组的垂直慧差较B组增高明显,两组间差异有统计学意义(P<0.05)。结论:W-G-LASIK和W-G-LASEK术后都会使术眼HOAs有不同程度的增加,总体来讲,W-G-LASEK的增加幅度小于W-G-LASIK。Objective To analyze the effects of coneal wavefront aberration-guided laser in situkeratomileusis （W-G-LASIK） and coneal wavefront aberration-guided laser epithelial keratomilersis （W-G-LASEK） on higher order aberrations （HOAs） in patients with myopia.Methods W-G-LASIK or W-G-LASEK for myopia was performed for 61 patients （122 eyes）,including W-G-LASIK （group A） for 27 patients （54 eyes） and W-G-LASEK （group B） for 34 patients （68 eyes）.HOAs of each eye were measured preoperatively and 7 days,1 month and 3 months postoperatively.The changes in HOAs before and after operation were analyzed to discuss the effects of W-G-LASIK and W-G-LASEK on HOAs.Results The root mean square （RMS） of the total HOAs in both groups showed an increase compared with that before operation （P〈0.05）,with dominant increases in the 3rs and 4th order aberrations and not obvious increases in the 5th and 6th order aberrations.Postoperatively,the total HOAs,3rd and 4th order aberrations in group B were lower than those in group A （P〈0.05）.The vertical coma,horizontal coma and spherical aberrations in both groups after operation were increased compared with those before operation （P〈0.05）,and the increase of vertical coma in group A was more significant than that in B group （P〈0.05）.Conclusion Both W-G-LASEK and W-G-LASIK increase the HOAs,but the HOAs increase of W-G-LASEK is less than that of W-G-LASIK.
