摘  要:目的 :对比研究自体富血小板血浆(platelet rich plasma,PRP)与神经生长因子(NGF)对面神经损伤的修复作用。方法:健康大耳白兔30只,随机分成3组,每组10只,选择一侧面神经上颊支横断后置于硅胶管神经再生室中,分别注入等量的PRP、NGF和生理盐水。术后8周分别进行面神经大体观察、神经电生理检测、组织学观察、图像分析,评价神经再生恢复情况。结果:电生理检测PRP组口轮匝肌动作电位潜伏期低于NGF组,且明显低于对照组(P<0.01);PRP组复合神经肌肉动作电位振幅(M波)高于NGF组,且明显高于对照组(P<0.01)。图像分析PRP组再生神经有髓轴突面积、髓鞘厚度及轴突计数高于NGF组,且明显高于对照组(P<0.01)。结论:PRP促进面神经损伤修复作用优于NGF。Objective: To study the regenerative potential between autologos platelet rich plasma (PRP) and nerve growth factor (NGF) on facial nerve regeneration. Methods: 30 healthy white rabbits were randomly divided into 3 groups (n=10 in each group). Cross section injury was performed at the buccal branch of facial nerve. Sectioned nerve ends were placed in silicone tube regenerative chamber. The same amount of PRP, NGF and physiological saline were injected respectively into the silicone tube. 8 weeks later, facial nerve gross observation, facial nerve electrophysiology detection, histological observation, image analysis, valuation of nerve regeneration recovery were detected. Results: Electrophysiological testing of the PRP group, orbicularis otis muscle action potential latencies was significantly lower than the experimental NGF group and the normal saline group (P〈0.01). The PRP's neuromuscular compound action potential amplitude (M wave) was higher than the NGF group and was significantly higher than the normal saline group (P〈0.01). Mage analysis results of regenerated nerve myelinated axons area, thickness of myelin sheath and axon counts of PRP group, was higher than that of NGF group, and obviously higher than that of normal saline group (P〈0.01). Conclusion: Nerve regenerative potential of PRP is better than that of NGF.
