Three Dimensional Miniplate Versus Standard Miniplate Fixation in the Management of Mandibular Fractures:A Systematic Review and Meta-analysis

摘  要:目的:本研究应用Meta分析评价不同的固定方法对治疗下颌骨骨折的疗效,并确定哪个治疗方案是下颌骨骨折的最佳选择。方法:计算机检索Pub Med、Embase等外文数据库及万方、中国知网、维普等中文数据库,检索时限为建库至2017年1月,搜集国内外有关不同内固定方法对下颌骨骨折治疗的研究,并采用Review Manager 5.2.6软件对所得数据进行Meta分析。结果:按照设计检索方法检索到967篇相关文献,18篇纳入文献,Meta分析结果示:三维微型接骨板较传统的内固定有显著的统计学意义。结论:三维、二维微型接骨板的对比显示,三维板降低术后并发症,且有显著的统计学意义,三维微型接骨板是下颌骨骨折治疗中的最佳选择。Objective The aims of the present study were to 1) evaluate clinical outcomes between different fixation methods in the management of mandibular fractures and 2) determine which fixation method is the best option for the treatment of mandibular fractures. Methods A systematic review was conducted according to PRISMA guidelines, examining Medline- Ovid, Embase, and Pubmed and Wan fang, CNKI databases .inclusion criteria were studies in humans, including randomized controlled trials, controlled clinical trials, and retrospective studies, with the aim of comparing the two techniques. In addition, the incidence of complications was evaluated. Results Eighteen publications were included: 10 randomized controlled trials, 3 controlled clinical trials,and 5 retrospective studies. 9 studies showed a low risk for bias, 4 studies showed a moderate risk for bias, and 5 studies showed a high risk for bias.There were statistically significant advantages for 3-dimensional miniplate. Conclusion The results of the meta-analysis showed lower complication rates with use of 3D plate compared with the use of 2 miniplates in the management of MFs, also this was statistically significant.
