晶状体不全脱位囊袋张力环植入术后远期效果观察

摘　　要：目的观察晶状体不全脱位超声乳化联合囊袋张力环植入术后的远期效果与并发症。方法行晶状体超声乳化联合囊袋张力环手术后随访3年以上的26例（28眼）晶状体不全脱位，分析术后的视力、屈光度、眼压、角膜内皮细胞密度、晶状体囊及人工晶状体位置等情况。结果术后3年最佳矫正视力≥0．5者25眼（89．29％），≥1．0者19眼（67．86％）。眼压正常，平均为（12．5±1．58）mmHg（1mmHg=0．133kPa）。角膜内皮细胞密度平均为（2248±110）个／mm2。2眼（7．14％）发生人工晶状体偏位。3眼（10．71％）出现局限性Soemmering环形成。12眼（42．86％）术后出现后发障，经激光后囊切开术后，视力明显改善。结论晶状体不全脱位施行超声乳化联合囊袋张力环植入术视力效果良好。Objective To observe the long-term efficacy and complications of phacoemulsification combined with capsular tension ring implantation in the treatment of subluxation of lens. Methods The data of 28 eyes of 26 cases with subluxation of lens were collected and analyzed retrospectively. They underwent capsular tension ring implantation combined with phacoemulsification . The follow up time was more than 3 years. The visual acuity , diopter , cell density of corneal endothelium, intraoeular pressure, the position of the IOL and lens capsula after surgery were observed. Results The best corrected visual acuity ≥0.5 were in 25 eyes（89.29% ）3 years after surgery; ≥1.0 were in 19 eyes （67.86%）. All cases of intraocular pressure were normal, mean（ 12.5 ±1. 58） mmHg（ ImmHg = 0. 133 kPa） . After surgery, the average corneal endothelium cell density were（2248 ± 110） mm2. IOL deviation happened in 2 eyes （7.14%）. Localized Soemmering ring formation happened in 3 eyes （ 10.71% ）. Twelve eyes （42.86%） with posterior capsular opacification, and the visual acuity was improved after laser capsulotomy. Conclusion Capsular tension ring implantation combined with phacoemulsification in lens subluxation is safe and reliable. The major later complication is posterior capsular opaeification.
