小切口角膜基质透镜取出术1000只眼并发症的观察

摘　　要：目的观察分析飞秒激光小切口角膜基质透镜取出术（SMILE）术中术后并发症。方法2013年2月至2016年6月进行SMILE手术矫正近视508例（1000眼），术中、术后对其并发症的情况进行回顾分析。结果术中并发症68例（73眼，7．3％）包括：负压吸引环脱失2眼（0．2％），不透明气泡层（OBL）27眼（2．7％），微切口出血15眼（1．5％），寻找角膜基质透镜困难11眼（1．1％），透镜分离困难5眼（0．5％），透镜撕裂3眼（0．3％），切口撕裂8眼（0．8％），扫描黑区2眼（0．2％）。术后并发症12例（14眼，1．4％）包括：轻度弥漫性板层角膜炎（DLK）2眼（0．2％），角膜帽下异物7眼（0．7％），过矫3眼（0．3％），欠矫2眼（0．2％）。结论SMILE手术治疗近视并发症少，安全有效，患者满意度高。Objective To observe the intraoperative and postoperative complications of femtosecond laser small incision lenticule extraction （SMILE）. Methods The data of 1 000 eyes of 508 patients with myopic received SMILE from Feb. 2013 to Jun. 2016 were collected. The intraoperative and postoperative complications were analyzed retrospectively. Results The intraoperative complications happened in 73 eyes of 68 cases（7.3% ） ,included suction ring lost in 2 eyes, opaque bubble layer（OBL） in 27 eyes（2.7% ）, incision bleeding in 15 eyes （1.5 % ）, failure find corneal stroma lens in 11 eyes （1.1% ）, lens separation difficulties in 5 eyes（0.5% ）, lens tear in 3 eyes（0.3% ）, incision laceration in 8 eyes （0.8%） and scanning blind area in 2 eyes （0.2%）. There were postoperative complications in 14 eyes of 12 cases （ 1.4% ） included mild diffuse lamellar keratitis（DLK） in 2 eyes（0.2% ）, foreign body under the cap in 7 eyes（0. 7% ）, over correction in 3 eyes（0.3% ） and under correction in 2 eyes（0. 2% ）. Conclusion The surgery of SMILE in the treatment of myopia is safe and effective. It has less complication and high satisfaction in patients.
