8例腮腺恶性淋巴瘤诊疗体会

摘　　要：目的:探讨腮腺恶性淋巴瘤的临床及辅助检查特点,协助提高术前诊断正确率。方法:本研究回顾性分析首都医科大学附属北京友谊医院口腔科于2013年9月~2016年5月收治的8例腮腺恶性淋巴瘤病历资料,并结合文献探讨腮腺恶性淋巴瘤的临床及辅助检查特点。结果:临床检查及CT检查表现为腮腺结节或肿块特别是伴有颈部淋巴结肿大并融合成团块;术前针吸活检结合免疫组化多能给出倾向性诊断;手术切除肿物,进行术后常规病理结合免疫组化分析,1例为霍奇金淋巴瘤,7例为非霍奇金淋巴瘤;术后均建议转入血液科或肿瘤科进一步治疗。结论:临床检查、CT检查、术前针吸活检结合免疫组化为腮腺淋巴瘤诊断提供依据,手术切除肿物明确病理分型指导其个性化治疗。Objective：To document the clinicopathologic features and accessory examination characteristics of malignant lymphoma of parotid gland for improving preoperative diagnosis rate.Methods：Eight cases of malignant parotid gland lymphoma admitted in the Department of Oral and Maxillofacial Surgery in Beijing Friend Hospital of Capital Medical University between September 2013 and May 2016 were reviewed.Results：The clinicopathologic features and CT image of primary malignant lymphoma of the parotid gland were parotid masses,associated with enlarged lymph nodes.Needle biopsy and immunohistochemistry can provide propensities to dignose.All eight patients received surgical treatment and got pathological diagnosis at last including 1case of Hodgkin＇S lymphoma and 7cases of non-Hodgkin＇S lymphoma.All patients were advised to hematological pepartment or oncological department for further treatment.Conclusion：The features of clinicopathology,CT image,needle biopsy,and immunohistochemistry can provide propensities to dignose primary malignant lymphoma of the parotid gland.The pathological diagnosis postoperative can direct individual therapy.
