经脐单孔腹腔镜阑尾切除术中阑尾系膜不同处理方法的临床研究

摘　　要：目的比较经脐单孔腹腔镜阑尾切除术中hem-0-lok夹法和电凝法处理阑尾系膜的优缺点。方法选取2014年4月至2015年4月航天中心医院收治的急性化脓性或坏疽性阑尾炎患者41例,采用随机数字法分为hem-0-lok组(20例)和电凝组(21例),分别按hem-0-lok夹法及电凝法处理阑尾系膜完成经脐单孔腹腔镜阑尾切除术,比较两组手术出血量、手术时间、住院时间、住院费用。结果 hem-0-lok组手术时间显著短于电凝组,住院费用显著高于电凝组[(57±15)min比(73±15)min,(10 405±1271)元比(8 466±873)元,P<0.01]。两组手术出血量、住院时间比较差异无统计学意义(P>0.05)。两组患者均未见严重手术相关并发症。结论对于处理经脐单孔腹腔镜阑尾切除术阑尾系膜两种方法均安全可靠,hem-0-lok夹法手术时间短,电凝法更具经济优势。Objective To compare the advantages and disadvantages of Hem-0-1ok clamp method and electrocoagnla- tion method on mesoappendix in transumbilical single-pert laparoscopic appendectomies. Methods From Apr. 2014 to Apr. 2015,41 cases of acute purulent and gangrenous appendicitis in Aerospace Center Hospital were divided into a hem- 0-1ok group of 20 cases and an electric coagulation group of 21 cases according to the random number table method,which received the transumbilical single-port laparoscopic appendectomy successfully by two differrent treatments of mesoappen- dix. The operative time, surgical blood loss, length of hospital stay, hospitalization costs were compared between the two groups. Results The operation time in the electric coagulation group was significantly shorter than that in the electric coag- ulation group[ （57 ± 15 ） min vs （73 ± 15） min] （P 〈 0. 01 ） ;The hospitalization cost in the hem-0-1ock group was signifi- cantly higher than that in the electrocoagnlation group[ （ 10 405 ± 1 273） yuan vs （8 466±873） yuan,P 〈0. 01 ]. The surgical blood loss and the length of hospital stay of two groups was no significant difference（P 〉0. 05）. There were no serious postoperative complications in both groups. Conclusion Both methods of treating the mesoappendix are safe and reliable. The operation time of hem-0-1ok methodis shorter, while the operation cost the electric coagulation methodis lower.
