摘 要:目的探讨宝石能谱CT最佳单能量技术优化甲状腺肿瘤供血动脉图像质量的价值。方法回顾性分析35例经手术和病理证实的甲状腺肿瘤患者,所有患者均在能谱模式下进行颈部双期增强扫描,扫描结束后机器自动重建出140kvp常规混合能量图像、70keV单能量图像,然后使用GSI Viewer软件获取最佳单能量图像。根据甲状腺肿瘤供血情况,共获取符合条件的甲状腺肿瘤供血动脉44支,采用单因素方差分析比较三组甲状腺肿瘤供血动脉的对比噪声比(CNR)、信噪比(SNR)及其主观评分。结果 44支甲状腺肿瘤供血动脉140kvp混合能量组、70keV单能量组和最佳单能量组的CNR分别为15.75±4.64、18.78±4.87和23.36±6.58;SNR分别为23.54±5.03、27.27±5.32和30.85±6.90;主观评分分别为3.13±0.51、3.77±0.63和4.36±0.52,差异均有统计学意义(F值分别为21.87、17.41和53.83,P值均<0.05),两两比较差异也均有统计学意义(P值均<0.05),最佳单能量组的CNR、SNR和主观评分均高于其余两组。结论能谱CT最佳单能量技术可以优化甲状腺肿瘤供血动脉CTA图像质量。Objective To investigate the value of the optimal monochromatic technology of gemstone spectral imaging in dispa- lying the supply artery of thyroid tumors. Methods 35 patients with thyroid tumors confirmed by surgery and pathology were ana- lyzed retrospectively, all the patients underwent the neck dual-phases contrast enhancement scanning with the GSI model. After scanning, the machine reeonstructed the 140 kVp mixed energy images, 70 keV single images automatically, and then the optimal monochromatic images were obtained by the GSI Viewer software. Finally, a total of 44 eligible supply arteries were involved, and the CNR, SNR, subjective scores of them in each groups were analyzed by one-way ANOVA method. Results CNR of the opti- mal monochromatic group, the 70 keV group, and the 140 kVp mixed energy group were 15.75 ±4.64, 18.78±4.87 and 23.36 ±6.58 respectively; SNR were 23.54 ± 5.03, 27.27± 5.32 and 30.85 ± 6.90 respectively; Subjective scores were 3.13 ± 0. 51, 3.77 ± 0.63 and 4.36 ± 0.52, respectively. CNR, SNR and subjective scores had significant difference among groups ( F = 21.87, 17.41 and 21.87, respectively, P 〈 0.05 ), and those in the optimal monochromatic group were superior to the 70 keV group and the 140 kVp mixed energy group. Conclusion The optimal monochromatic mode can improve the CTA quality of the thyroid tumor supply arteries.
