摘 要:目的探讨乳腺粘液腺癌的声像图特点,分析超声误诊的原因。方法对病理确诊的15例乳腺粘液腺癌患者的超声影像及临床资料进行回顾性分析。结果本组超声误诊率为80%,其中8例超声诊断为良性结节,4例诊断为乳腺纤维瘤,仅3例诊断为乳腺粘液腺癌。超声图像表现为:形态规则的占93.3%(14/15)、边界清晰的占73.3%(11/15)、93.3%(14/15)的肿块后方回声增强、60%(9/15)为低回声,40%(6/15)为等回声,46.7%(7/15)的肿块回声均匀,53.3%(8/15)的肿块内部呈条带状分布不均、另有26.7%(4/15)肿块内见液性区,腋下淋巴结均无明显增大。13.3%(2/15)的病灶内动脉血流阻力指数超过0.8。结论乳腺粘液腺癌常规超声表现具有一定的特征性,但易误诊为纤维瘤等良性肿瘤,需仔细观察鉴别。Objective To discuss the uhrasonographic features of mucinous carcinoma of breast and to analyse the causes of misdiagnosis of ultrasound. Methods The ultrasound imaging and clinical data of 15 patients were retrospectively analyzed, who were diagnosed as mucinous carcinoma of breast by pathology. Results The ultrasound misdiagnosis rate of this group was 80%. Among them, the 8 cases were diagnosed as benign nodules by ultrasound, 4 cases were diagnosed as mammary gland fibroma, only 3 cases were diagnosed as mucinous carcinoma of breast. Ultrasound images showed that morphological rules accounted for 93.3% and clear boundary accounted for 73.3%. 93.3% ( 14/15 ) of the mass was with posterior echo enhancement. 60% ( 9/ 15) was hypoechoic. 40% (6/15) was equal echo. 46.7% (7/15) of the mass echo uniform. Internal distribution was uneven strip for 53.3% (8/15) mass. The liquid region within 26.7% (4/15) mass was seen. There was no significant increase in the axillary lymph nodes. Within 13.3% (2/15) of lesions, artery resistance index was over O. 8. Conclusion The general ultrasound breast mucinous carcinoma has a certain characteristic performance, but was easily diagnosed as benign fibroma by mistakenly. Careful observation and identification is needed.
