58例结外NK/T细胞淋巴瘤治疗和预后分析

摘　　要：目的分析结外NK/T细胞淋巴瘤近远期疗效影响因素。方法 2011年2月—2016年7月,该院收治结外NK/T细胞淋巴瘤58例,单纯放疗4例、单纯化疗18例、放化疗联合治疗36例,常规随访、复查,评价近期疗效、远期生存情况。结果完全缓解22.41%、部分缓解39.66%、稳定22.41%、进展15.52%,总有效率79.31%、控制率68.59%;有效与无效者Ann ArborⅢ-Ⅳ、β2微球蛋白升高、LDH水平升高、B症状比重差异有统计学意义(P<0.05);1年生存率为68.63%(35/51),2年生存率为55.81%(24/43);2年内生存24例纳入生存组,死亡患者纳入死亡组,两组有局部肿瘤侵犯、IPI>2分、Ann ArborⅢ-Ⅳ、LDH>240 U/L占比差异有统计学意义(P<0.05),多因素分析均成为独立危险因素。结论结外NK/T细胞淋巴瘤近远期预后均受多种因素影响。Objective To analyze the influence factor of long-term and short-term curative effect of lymphoma extran- odal NK/T cell. Methods 58 cases of patients with extranodal NK/T-cell lymphoma admitted and treated in our hospi- tal from February 2011 to July 2016 were selected, simple radiotherapy was in 4 cases, simple chemotherapy was in 18 cases and combined treatment of radiotherapy and chemotherapy was in 36 cases, and the patients were given the rou- tine follow-up and reexamination, and the short-term curative effect and long-term survival situation were evaluated. Results Complete remission was in 22.41%, partial remission was in 39.66%, 22.41% was steady and progress was in 15.52%, and the total effective rate was 79.31% and control rate was 68.59%, and the differences in the Ann ArborⅢ- Ⅳ, β2 microglobulin increase, LDH level increase and B symptom ratio between the ineffective persons and effective persons were statistically significant P〈0.05, the 1-year survival rate was 68.63%（35/51） and 2-year survival rate was 55.81%（24/43）, and the 24 survival cases in 2 years were included into the survival group, and the death patients were included into the death group, and local tumor invasion was in both groups, and the differences in the ratios of IPI〉2 scores, Ann ArborⅢ- Ⅳ and LDH〉240 U/L were statistically significant P〈0.05, and all became the independent risk factors by multi-factor analysis. Conclusion The short-term and long-term prognosis of extranodal NK/T-cell lym- phoma is affected by multiple factors.
