摘 要:目的将患者的CT影像转化为数字模型文件,应用成熟的3D打印技术,合成患者病变部位解剖、颜色等相同的模型,通过模拟合成来预防术中突发状况以及术后并发症的发生。方法选择2015年6—12月在该院收治的复杂肝胆管结石病患者42例,根据患者是否自愿行3D打印技术,分为观察组和对照组,分别给予3D打印技术辅助外科手术治疗和常规的CT影像学资料辅助外科治疗,对比两组患者的临床相关指标、结石残留率以及并发症的发生情况。结果观察组患者在手术时间、术中出血量、术中输出血量、住院天数以及肝脏血流阻断时间等方面和对照组相比较差别显著;观察组患者的即刻结石残留率、最终残留率分别为8.33%、4.17%,对照组患者分别为44.44%、27.78%,差异有统计学意义(P<0.05);观察患者并发症发生率为4.17%,对照组发生率为38.89%,差异有统计学意义(P<0.05)。结论 3D打印技术辅助复杂肝胆管结石术可以提高手术的完成时间、减少出血量,同时减少并发症的发生率加快患者的康复,值得在临床广泛应用。Objective To prevent the intraoperative accidents and occurrence of postoperative complications by apply- ing the mature 3D print technology after converting the CT imaging into the digital model document. Methods 42 cases of patients with hepatolith complicated disease admitted and treated in our hospital from June 2015 to December 2016 were selected and divided into two groups according to whether the patients were voluntary or not, the observation "group adopted the 3D print technology and surgical operation treatment, while the control group adopted the routine CT imaging data and surgical treatment, and the clinical related indexes, stone residual rate and occurrence of complica- tions were compared between the two groups. Results The differences in the operation time, intraoperative bleeding amount, intraoperative blood transfusion amount, length of stay and hepatic vascular occlusion time between the two groups were obvious, and the differences in the immediate stone residual rate, final residual rate and incidence rate of complications between the observation group and the control group were obvious with statistical significance (8.33%, 4.17%, 4.17% vs 44.44%, 23.38%, 38.89%)(P〈0.05). Conclusion 3D print technology and hepatolith complicated dis- ease operation can reduce the operation time, reduce the bleeding amount and incidence rate of complications and speed up the recovery of patients, which is worth wide application in clinic.
