摘 要:目的探讨软骨肉瘤组织中Fascin蛋白的水平变化及临床意义。方法选取2009年1月至2016年1月间南京医科大学第一附属医院收治的92例软骨肉瘤患者为观察组,选取同期67例软骨瘤患者为对照组。采用免疫组化EnlivionTM法分别检测两组患者组织中Fascin蛋白表达,并分析Fascin蛋白水平变化与软骨肉瘤临床病理特征之间的关系。结果软骨肉瘤组织中的Fascin蛋白阳性率为70.7%,软骨瘤组织中为4.5%,两组比较,差异有统计学意义(P<0.05)。软骨肉瘤组织中,Fascin蛋白阳性率与患者年龄、性别无显著相关性,差异无统计学意义(P>0.05);在不同病理类型中,间叶型、透明细胞型与普通型组织的Fascin蛋白阳性率表达无明显差异,但去分化型软骨肉瘤Fascin蛋白阳性率91.7%明显高于普通型的78.9%,组间差异有统计学意义(P<0.05);在普通型软骨肉瘤组织学分级中,Ⅰ级阳性表达率为47.6%,Ⅱ级为63.2%,Ⅲ级为88.2%,组间差异有统计学意义(P<0.05);Fascin蛋白在肺转移组中的表达为87.2%,明显高于无肺转移组的48.9%,组间差异有统计学意义(P<0.05)。结论 Fascin蛋白参与了软骨肉瘤恶变的发生、发展过程,其高表达水平与软骨肉瘤的分化、转移密切有关,可作为临床治疗软骨肉瘤的新方向。Objective To explore changes in protein levels of fascin in chondrosareoma and the significance. Methods From January 2009 to January 2016, 159 patients treated at the First Affiliated Hospital of Nanjing Medical University were selected, among which 92 patients with chondrosarcoma were selected as the observation group and 67 patients with chondroma as the control group. Using immunohisto- chemical EnlivionTM assay, expression of of fascin was detected in both groups and the relationship between changes in protein levels of fascin and clinicopathological features of chondrosarcoma was analyzed. Results Positive rate of fascin was 70. 7% in tissues of chondrosarcoma and 4. 5% in tissues of chondroma( P 〈 0. 05 ). The positive rate of fascin in tissues of chondrosarcoma was not significantly correlated with age and gender of patients ( P 〉 0. 05 ). Among different pathological types, there was no statistically significant difference in positive rate of fascin among tissues of mesenchymal chondresarcoma and clear cell type chon- drosarcoma and ordinary chondrosarcoma ( P 〉 0. 05 ). However, the positive rate of fascin for differentiated chondrosarcoma (91.7%) was significantly higher than for ordinary chondrosareoma (78.9%) (P 〈 0. 05). For normal histologic grading of chondrosarcoma, positive expression rate was 47. 6% for grade I chondrosarcoma, 63.2% for grade lI and 88.2% for grade III (level was 88.2% (P 〈0. 05). Expressionrate of fascin protein was 87.2% for lung metastasis compared with 48. 9% for non-lung metastasis (P 〈 0. 05 ). Conclusion Fascin protein plays an important role in the occurrence and development of chondrosarcoma and its high expression level is associated with differentiation and metastasis of chondrosar- coma, which can be used as a new direction in the clinical treatment of chondrosarcoma.
