摘 要:目的探讨术中实时超声指导对颅内胶质瘤手术的应用价值。方法选取2013年5月至2015年10月间湖北省天门市第一人民医院收治的63例颅内胶质瘤患者,均通过术中实时超声指导实施肿瘤切除手术。术后三天,对患者采取强化MRI,复查肿瘤切除范围,评估手术准确性及全切率。结果 63例患者术中肿瘤发现率和定位准确率均为100.0%。有61例患者术中超声探查肿瘤范围和术后强化MRI复查结果一致,总体准确率为96.8%。53例接受肿瘤全切手术的患者中,52例达到全切效果,肿瘤全切率为98.1%。所有患者经手术治疗后病情均得到明显改善。结论术中实时超声指导,有助于提高颅内胶质瘤手术的全切率和准确率,值得临床推广。Objective To explore the role of real-time intraoperative ultrasound guidance in in- tracranial glioma surgery. Methods From May 2013 to October 2015, 63 patients with brain glioma who were treated at Tianmen First People's Hospital were selected. All the patients underwent intracranial glioma surgery with real-time intraoperative ultrasound guidance. At 3 days after the surgery, enhanced MRI was used to assess the extent of surgical resection, accuracy of operation and full cut rate. Results Both detec- tion and positioning accuracy were 100%. Of the 63 patients, consistence of accuracy between intraopera- rive ultrasound tumor range and postoperative enhanced MRI reexamination was found in 61 patients and the overall accuracy was 96. 8%. Of 53 patients undergoing total tumor resection, 52 patients achieve efficacy of total resection and total tumor resection rate was 98.1%. For all the patients, the disease were obviously improved after the surgical treatment. Conclusion Real-time intraoperative ultrasound guidance in in- tracranial glioma surgery helps to improve the overall removal rate and accuracy of the surgery which is wor- thy of application.
