弥漫大B细胞淋巴瘤中医阳虚体质评分与LDH的关系

摘　　要：目的探讨初发弥漫大B细胞淋巴瘤(DLBCL)中医体质评分与乳酸脱氢酶(LDH)的关系,为中医治疗DLBCL提供可能的依据。方法以初诊DLBCL患者为研究对象,采集临床资料,根据2010版NCCN指南《非霍奇金淋巴瘤临床实践指南》判定国际预后指数(International Prognostic Index,IPI);采集中医体质量表,依据2009年中华中医药学会颁布《中医体质分类与判定》判定患者发病前的中医体质及各中医体质类型评分。采用SPSS20.0统计学软件,运用多元线性回归统计学方法分析体质评分、IPI及LDH三者间的线性关系。结果 (1)阳虚体质评分与LDH成线性正相关,回归系数为1.358,差异有统计学意义(P<0.05);(2)LDH与IPI成线性正相关,回归系数为82.686,差异有统计学意义(P<0.01);(3)IPI与中医各体质评分无线性相关。结论以中医体质评分系统进行判定,阳虚评分越高,LDH值越高,提示阳虚体质评分可能与DLBCL后肿瘤负荷的大小有关,但中医体质评分与IPI无关,尚不能作为评估DLBCL预后的参数。Objective To explore the relationship between TCM yang deficiency body constitution and lactate dehydrogenase（LDH）in diffuse large B-cell lymphoma（DLBCL）and provide the potential evidences for the treatment of DLBCL with TCM.Methods Taking the first-visited DLBCL patients as the subjects,collecting clinical data and in accordance with NCCN Clinical practice guideline for non-Hodgkin＇s lymphoma,version 2010,the international prognostic index（IPI）was assessed.Collecting TCM constitution scale,in accordance with Classification and assessment of TCM constitution issued by China Association of Chinese Medicine,the TCM constitution before sickness and the score of every TCM constitution type were judged.Adopting SPSS20.0 software,using multiple linear regression analysis,the linear relationship among the constitution score,IPI and LDH were analyzed.Results 1.The positive linear correlation presented between yang deficiency constitution score and LDH.The regression coefficient was 1.358,indicating the significant difference（P〈0.05）.2.The positive linear correlation presented between IPI and LDH.The regression coefficient was 82.686,indicating the significant difference（P〈0.01）.3.There was no linear correlation between IPI and each TCM constitution score.Conclusion TCM constitution scoring system is used for the judgment.The higher the yang deficiency score is,the higher LDH value is.It is suggested that yang deficiency constitution score is probably related to tumor load after DLBCL.TCM constitution score is not correlative with IPI,which cannot be taken as the parameter for the evaluation of DLBCL prognosis.
