代谢综合征对急性胰腺炎的影响

摘　　要：[目的]研究代谢综合征对急性胰腺炎病情发展的影响及预后。[方法]回顾分析290例急性胰腺炎患者的临床资料,根据其是否伴有代谢综合征(MS),分为MS组(140例)及非MS组(150例),将2组患者临床疗效、C反应蛋白、血钙、血氧分压、住院时间、重症胰腺炎的发生率、Ranson评分、APACHE II评分、局部及全身并发症以及死亡的发生率进行比较。[结果]MS组较非MS组临床疗效差,C反应蛋白高,血钙,血氧分压低,住院时间长(均P<0.05),SAP的患病率、Ranson评分、APACHE II评分高(P<0.01),局部及全身并发症高(P<0.01)以及死亡发生率高(P<0.05),差异均有统计学意义。[结论]MS影响着急性胰腺炎的病情、分型及预后,临床对急性胰腺炎患者是否存在MS的评估可作为判断急性胰腺炎病情严重程度、预后的客观、快速的评价指标之一,值得临床推广应用。[Objective]To investigate the effect and prognosis of metabolic syndrome on progression of acute pancreatitis. [Methods]Total 290 patients suffering acute pancreatitis who were admitted to the hos- pital from January 2008 to March 2016 were divided into acute pancreatitis with metabolic syndrome（n= 140, MS group）and acute pancreatitis without metabolic syndrome（n= 150,non-MS group）. The treatment effects,CRP, serum calcium level, PaO2, hospital stays, SAP incidence rate, Ranson grade, APACHE II grade,the local and system complications and mortality rate in the two groups were compared. [-Results] Compared with the non-MS group,the treatment effect in MS group was poor,CRP was higher, serum cal- cium,PaO2 were lower,hospital stays was longer（t=4.54 and 4.87 and 4.67 and 3.68,P〈0.05） ,the SAP incidence rate, Ranson grade and APACHE II grade were higher（x^2= 5.38, P%0.01）, the local and system complications（P〈0.01）and mortality incidence rate were higher（P〈0.05）,all the differences had statisti- cal significance. [Conclusion] Metabolic syndrome affects the symptoms, classification and prognosis of a- cute pancreatitis. Clinical evaluation of presence of metabolic syndrome in patients with acute pancreatitis, can be used on objectively estimating the prognosis of acute pancreatitis and worthy of clinical application.
