牙周炎患者口腔Hp分布特点及与胃Hp感染的关系

摘　　要：目的探讨牙周炎患者口腔幽门螺杆菌(Hp)的分布特点及与牙周炎、胃Hp感染的关系,为临床治疗提供参考依据。方法选择医院2014年1月-2015年12月慢性牙周炎患者150例作为牙周炎组和牙周健康者150例作为对照组,牙周炎组分为轻度牙周炎患者93例、中度牙周炎患者31例,重度牙周炎患者26例;检测口腔Hp和胃Hp感染情况。结果牙周炎组口腔Hp阳性率为82.7%显著高于对照组48.0%(P<0.05);牙周炎组胃Hp阳性率为85.3%高于对照组43.3%(P<0.05);轻度牙周炎患者、中度牙周炎患者、重度牙周炎患者口腔Hp阳性率分别为78.5%、87.1%、92.3%均高于对照组患者48.0%(P<0.05);重度牙周炎患者口腔Hp阳性率高于中度牙周炎患者,中度牙周炎患者口腔Hp阳性率高于轻度牙周炎患者,但比较差异无统计学意义;口腔Hp阳性患者胃Hp阳性率为91.1%高于口腔Hp阴性患者57.7%(P<0.05)。结论慢性牙周炎患者口腔Hp阳性率高,口腔Hp感染和牙周炎及胃Hp感染关系密切。OBJECTIVE To investigate the distribution characteristics of Helicobacter pylori （Hp） in patients with periodontitis and its relationship with periodontitis and gastric Hp infection, so as to provide references for clinical treatment.METHODS A total of 150 patients with chronic periodontitis from Jan.2014 to Dec.2015 were selected as periodontitis group and 150 cases of periodontal health were selected as control group.The chronic periodontitis patients were randomly divided into mild periodontitis group （93 cases）, moderate periodontitis group （31 cases） and severe periodontitis group （26 cases）.Oral Hp and gastric Hp infection were detected.RESULTS The positive rate of oral Hp in periodontitis group was 82.7%, which was significantly higher than 48.0% in control group （P〈0.05）.The positive rate of gastric Hp in periodontitis group was 85.3%, which was higher than 43.3% in control group （P〈0.05）.The positive rates of oral Hp in mild periodontitis group, moderate periodontitis group and severe periodontitis group were 78.5%, 87.1% and 92.3%, which were higher than 48.0% of control group（P〈0.05）.The positive rate of oral Hp in severe periodontitis group was higher than that in moderate periodontitis group, that in moderate periodontitis group was higher than that in mild periodontitis group, but the differences were not significant.The positive rate of gastric Hp in oral Hp positive group was 91.1%, which was higher than 57.7% in oral Hp negative group （P〈0.05）.CONCLUSION The oral Hp positive rate in chronic periodontitis patients is high , and oral Hp infection is closely related to periodontitis and gastric Hp infection.
