摘 要:目的探索研究肿瘤坏死因子-α(TNF-α)、环氧化酶-2(COX-2)、P53以及细胞角蛋白7(CK7)在Barrett食管(BE)中的表达情况,分析其变化的意义。方法 38例明确BE诊断患者,通过免疫组织化学对BE患者术后组织进行切片染色,将患者BE病变区域组织设为BE组(42例),将BE边缘食管炎部分组织作为食管炎组(27例),将BE病变部位外5 cm正常食管黏膜作为正常组(29例)。比较三组TNF-α、COX-2、P53及CK7的表达情况。结果正常组TNF-α阳性率为20.7%,食管炎组为92.6%,BE组为81.0%,正常组TNF-α阳性率低于食管炎组、BE组,差异具有统计学意义(P<0.05);食管炎组和BE组TNF-α阳性率比较差异无统计学意义(P>0.05)。正常组COX-2阳性率低于食管炎组、BE组,差异具有统计学意义(P<0.05);食管炎组和BE组COX-2阳性率比较差异无统计学意义(P>0.05)。正常组P53蛋白阳性率低于食管炎组、BE组,差异具有统计学意义(P<0.05);BE组P53蛋白阳性率高于食管炎组,差异具有统计学意义(P<0.05)。正常组、食管炎组CK7蛋白阳性率比较差异无统计学意义(P>0.05),BE组CK7蛋白阳性率均高于正常组和食管炎组,差异具有统计学意义(P<0.05)。结论 TNF-α、COX-2、P53及CK7与BE的发生具有密切联系,通过检测其表达量能够对BE诊断提供一定指导意义。Objective To explore and study the expression of tumor necrosis factor- α (TNF- α), cyclooxygenase-2 (COX-2), P53, cytokeratin 7 (CKT) in Barrett esophagus (BE), and analyze the significanceof their changes. Methods A total of 38 confirmed BE patients, and their postoperative tissue was stained by immunohistoehemistry. The BE lesion tissue was taken as group BE (38 eases), the BE marginal esophagitis tissue was taken as esophagitis group (27 eases), and the normal esophageal mueosa of 5 em outside the site of BE lesion was taken as normal group (29 eases). Comparison were made on expression of TNF-cx, COX-2, P53 and CK7 in three groups. Results The normal group had TNF- α positive rate as 20.7%, which was 92.6% in esophagitis group and 8,1.0% in BE group. The normal group had h)wer TNF- α positive rate than esophagitis group and BE group, and their difference had statistical significance (P〈0.05). The esophagilis group and BE group had no no statistically significant difference in TNF- α positive rate (P〉0.05). The normal group had lower COX-2 positive rate than the esophagitis group and BE group, and their difference had statistical significance (P〈0.05). The esophagitis group and BE group had no no statistically significant difference in COX-2 positive rate (P〉0.05). The normal group had lower P53 protein positive rate than the esophagitis group and BE group, and their difference had statistical significance (P〈0.05). The BE group had higher P53 protein positive rate than the esophagitis gronp, and the difference had statistical significance (P〈0.05). The normal group and esophagitis group had no statistically significant difference in CK7 protein positive rate (P〉0.05). The BE group had higher CK7 protein positive rate than the normal group and esophagitis group, and the difference had statistical significance (P〈0.05).
