摘  要:目的探讨社区卫生服务质量对高血压患者管理效果的影响。方法基于基本医疗质量评估量表(Primary Care Assessment Tools,PCAT)从患者角度对社区卫生服务质量进行评价,采用高血压患者知识评分、行为评分、收缩压和舒张压4个指标对高血压管理效果进行评价。应用Pearson相关和广义线性模型分析患者PCAT评分与高血压管理效果评价指标之间的关系。结果调查的726名患者平均年龄为65.8±10.2岁,患者PCAT评分、知识评分、行为评分、收缩压和舒张压分别为29.53,7.52,8.16,138.25和84.47。采用广义线性模型控制患者基本情况后,PCAT评分与患者知识评分和行为评分呈正相关,与患者收缩压和舒张压呈负相关,回归系数分别为0.19,0.22,-2.15和-1.07,研究结果具有统计学意义(P<0.05)。结论患者对社区卫生服务质量的认可对高血压管理效果有积极作用,有必要从首诊、持续性、协调性、综合性和社区为导向5个维度促进社区卫生服务的发展和服务质量的提升。Objectvies To explore the effect of community health service quality on management of hypertensvie patients.Methods The Primary Care Assessment Tool(PCAT)was used as the patient assessing tool to assess the quality of primary care provided by the community health service center.Hypertensvie patients&#39; knowledge score,health behavior score,systolic blood pressure and diastolic blood pressure were used to evaluate the effect of hypertension management.Pearson correlation and generalized linear model were adopted to identify the relationship between PCAT score and hypertension management indicators.Results The average age of the 726 hypertensvie patients was 65.8 years.Hypertensvie patients&#39; PCAT score,knowledge score,health behavior score,systolic blood pressure and diastolic blood pressure were 29.53,7.52,8.16,138.25 and 84.47,respectviely.PCAT score was positviely correlated with patient&#39;s knowledge score and health behavior score,and was negatviely correlated with systolic and diastolic blood pressure,the regression coefficients were 0.19,0.22,-2.15 and-1.07,respectviely.Conclusions Patient recognition of community health service quality has a positvie effect on hypertension management.It is necessary to promote the development of community health services and the improvement of service quality from fvie dimensions:first contact,continuity,coordination,comprehensvieness and community orientation.
