视神经脊髓炎患者血清NMO-IgG、hs-CRP及触珠蛋白的水平变化及其意义

摘　　要：目的探讨视神经脊髓炎(NMO)患者血清水通道蛋白4特异度结合的自身抗体-抗水通道蛋白4抗体(NMO-IgG)、超敏C反应蛋白(hs-CRP)、触珠蛋白的水平变化及其意义。方法选取2011年1月至2016年12月确诊的NMO患者54例(A组)、长节段横惯性脊髓炎患者31例(B组)、多发性硬化患者31例(C组)进行回顾性分析,检测三组患者血清NMO-IgG、hs-CRP、触珠蛋白水平并进行比较分析。结果急性期,血清NMO-IgG水平比较,A组高于B组、C组(P<0.05),B组高于C组(P<0.05);急性期,三组患者的血清hs-CRP、触珠蛋白水平比较,差异均无统计学意义(P>0.05);NMO患者急性期血清NMO-IgG、hs-CRP、触珠蛋白水平、EDSS评分均显著高于缓解期,差异具有统计学意义(P<0.05);急性期,NMO患者EDSS评分与血清NMO-IgG、hs-CRP、触珠蛋白水平均呈显著正相关关系(P<0.05);缓解期,NMO患者EDSS评分与血清NMO-IgG、触珠蛋白水平均呈显著正相关关系(P<0.05),NMO患者EDSS评分与hs-CRP的相关性无显著性(P>0.05)。结论 NMO患者血清NMO-IgG、hs-CRP、触珠蛋白的水平变化与患者病情程度具有一定的关系,检测上述指标对于指导临床治疗具有一定的价值。Objective To study neuromyelitis optica （NMO） water channel protein 4 in serum of patients with specific antibodies of anti aquaporin 4 antibody （ NMO - IgG）, high sensitive C reactive protein （ hs - CRP）, changes in the levels of keratin protein and its significance. Methods From January 2011 to December 2016, 54 NMO patients （ A group）, 31 patients with long segment inertia transverse myelitis （ group B）, 31 patients with multiple sclerosis （group C） were retrospectively analyzed. The serum NMO - IgG, hs - CRP, keratin protein level were de- tected and compared. Results In acute phase, serum NMO - IgG levels in group A were higher than those in group B and group C （ P 〈 0.05 ） , while those in group B were higher than those in group C （ P 〈 0.05） ; In acute stage, serum hs - CRP, haptoglobin in three groups showed no significant difference （ P 〉0.05）. The serum NMO - IgG, hs - CRP, haptoglobin, EDSS scores in group A were significantly higher than those in remission, the difference was statistically significant （ P 〈 0.05 ）. EDSS score and serum NMO - IgG, hs - CRP and haptoglobin in acute NMO patients, were significantly positively correlated （ P 〈 0.05 ）. EDSS score and serum NMO - IgG and haptoglobin levels in remission NMO pa- tients were significantly positively correlated （ P 〈 0.05 ）. The correlation between NMO score and bs - CRP EDSS were not significant （ P 〉 0.05 ）. Conclusion The levels of serum NMO - IgG, hs - CRP and keratin in patients with NMO have a certain relationship with the severity of the disease, which has value in guiding clinical treatment.
