摘　　要：目的探讨结构化团体怀旧疗法对改善社区老年卒中后抑郁患者的影响。方法采用汉密尔顿抑郁量表(HAMD),根据随机抽样原则,对成都市新都区、金牛区、青羊区6家社区卫生服务中心提供的老年人健康档案既往病史中有"脑卒中或脑中风"的康复期患者,进行问卷调查。筛选出符合要求的老年卒中后轻、中度抑郁患者86例,按照随机数字表法,分为对照组和干预组,每组43例。对照组患者进行社区常规健康教育;干预组患者开展结构化团体怀旧的治疗方案,干预时间为6周。干预前、干预结束时分别对两组卒中后患者的抑郁程度进行评估,并对两组实施效果进行对比分析。结果干预组的HAMD抑郁评分在干预后明显降低,且低于对照组,差异有统计学意义(P<0.05);干预组抑郁症状的缓解率明显优于对照组,差异有统计学意义(P<0.05)。结论结构化团体怀旧疗法有助于缓解社区老年人卒中后抑郁,提高患者心理健康的水平。Objective To explore the effect of the structured group reminiscence therapy on the elderly patients with post stroke depression in community. Methods Hamilton depression rating scale （HAMD） was adopted to make a questionnaire survey on the basis of the principle of random sampling among the community-dwelling elderly patients with the medical history of stroke according to the medical records provided by the secondary or tertiary hospitals and the health records provided by the community health service center. 86 cases with light or moderate depression were selected and divided into the control group and intervention group according to the random number table, and each group consisted of 43 cases. The control group was given the conventional health education, while the intervention group was treated with the structured group reminiscence therapy. The intervention lasted for 6 weeks. The depression degree was evaluated before and after intervention respectively and the intervention effect was analyzed and compared between the two groups. Results The depression score of HAMD in the intervention group was reduced obviously and it was significantly lower than that in the control group after intervention （P〈0. 05）; The remission rate of depressive symptoms in the intervention group was significantly better than that in the control group （P〈0. 05）. Conclusion The structured group reminiscence therapy was helpful in easing the post stroke depression of the elderly patients in community and raising the level of their mental health.
