经直肠腔内超声对直肠癌术前分期准确性的评估与分析

摘  要:目的:评估经直肠腔内超声(ERUS)用于经肛内镜下微创外科(TEM)术前分期中的准确性。方法:对2011年1月-2014年12月在本院治疗的早期直肠癌拟行TEM切除的110例患者进行ERUS检查,并且与术后的病理学报告和内镜检查结果进行比较和分析。结果:ERUS的UT分期与病理学报告的PT分期符合率为96.4%。Friedman检验显示,ERUS与病理学报告比较,差异无统计学意义(P>0.05)。Kendall’s检验和Spearman相关性分析显示,ERUS与术后组织病理学检查结果存在显著的相关性(P<0.01)。通过ERUS测量的肿瘤位置与经内镜测量的位置比较,差异无统计学意义(P>0.05)。结论:ERUS能够较准确地判断早期直肠癌的分期和位置,可以为TEM手术的实施提供一定的依据。Objective:To evaluate the accuracy of transrectal ultrasound(ERUS) for colorectal cancer preoperative staging before endoscopic minimally invasive surgery(TEM).Method:A total of 110 patients with early stage colorectal cancer who were admitted to the department of general surgery in our hospital from January 2011 to December 2014 were recruited in this study.All patients underwent ERUS and TEM,the results from ERUS were analyzed and compared with the results from postoperative histopathological examination and endoscopy.Result:The agreement rate between ERUS UT staging and histopathological PT staging was 96.4%.Friedman test showed that there was no significant difference in ERUS and pathological results(P〉0.05).Kendall's test and Spearman correlation analysis showed that there was a significant correlation in ERUS and postoperative histopathological findings(P〈0.01).In addition,there was no significant difference in regarding the position and location of the tumor of ERUS and endoscopic measurements(P〉0.05).Conclusion:ERUS can more accurately determine the stages and positions of early stage colorectal cancer and can provide a valuable reference for the implementation of TEM surgery.
