射频热疗联合胸腔灌注药物治疗乳腺癌致恶性胸腔积液的临床观察

摘　　要：目的:观察射频热疗联合胸腔灌注华蟾素、顺铂对乳腺癌所致恶性胸腔积液患者的临床疗效和不良反应。方法:62例确诊为乳腺癌伴恶性胸腔积液患者随机分为两组,热疗组32例患者采用射频热疗联合华蟾素与顺铂胸腔灌注治疗,对照组30例患者仅采取华蟾素与顺铂胸腔灌注治疗,治疗进行一月后对两组患者进行近期疗效率及Kamofsky(KPS)评分对比研究。结果:热疗组近期疗效总有效率为87.5%,优于对照组的66.7%;热疗组KPS提高率(93.8%)明显高于对照组(70.0%),差异均具有统计学意义(P<0.05)。两组不良反应发生率差异无统计学意义。结论:射频热疗联合胸腔灌注顺铂和华蟾素明显提高了乳腺癌致恶性胸腔积液的治疗效果,患者的生存质量得到明显改善,是一种具有临床应用价值的治疗方法。Objective： To observe the therapeutic effect and adverse reaction of hyperthermia combined with intra- pleural infusion in the treatment of malignant pleural effusion caused by breast cancer. Methods： Sixty-two breast cancer patients with malignant pleural effusion were randomly divided into hyperthermia group and control group. Thirty-two pa- tients in the hyperthermia group received radiofrequency hyperthermia in combination with intrapleural infusion of cinobufa- cini and eisplatin. Thirty patients in the control group were only treated with cinobufagin and cisplatin by intrapleural infu- sion. One month after the therapy, the effective rate and the KPS score of two groups were analyzed by statistical compara- tive study. Results： The total short-term effective rate of the hyperthermia group was 87.5 %, which was significantly higher than that of the control group （ 66.7% ）. The KPS score increasing rate was 93.8% in the hyperthermia group and 70.0% in the control group with significant difference between these two groups （ P 〈 0.05 ）. No significant difference was observed in the incidence of adverse reaction between these two groups. Conclusion： Hypertherrnia combined with intrapleural infu- sion with cinobufacini and cisplatin can increase the treatment efficacy of breast cancer with malignant pleural effusion and improve the patient＇s quality of life significantly. It is worthy of clinical application.
