对精神科老年住院患者用药情况的分析

摘　　要：目的 :分析精神科老年住院患者的用药情况,旨在提高其用药的安全性。方法 :对2015年4月至2016年7月期间在青岛市精神卫生中心住院治疗的112例老年精神疾病患者的临床资料进行回顾性分析。调查这112例患者的用药情况,分析其所使用抗精神病药物的种类和剂量、联合用药的情况及使用其他药物的情况。结果:这112例患者所使用的抗精神病药物主要为奥氮平、喹硫平、阿立哌唑及氟西汀等,均为小剂量用药。其中,以奥氮平和喹硫平的使用率为最高,分别为42.9%和35.7%。在这些患者中,有89例患者为单一用药,有23患者为联合使用两种药物。没有联合使用三种药物的患者。在其他药物的使用方面,格列本脲和二甲双胍等降糖药的使用率较高,分别为40.2%和32.1%。心可舒和卡托普利等降压药的使用率也较高,分别为23.2%和42.9%。结论:奥氮平、喹硫平及阿立哌唑是临床上使用较为广泛的三种抗精神病药物,其具有疗效好、安全性高、患者耐受性佳等优点。老年人由于自身的生理功能下降,机体对药物的适应性较差,故其单一用药的安全性更高。对联合使用抗精神病药物和降压药、降糖药的精神科老年住院患者,临床上应密切监测其身体状况,以免其发生药物不良反应。Objective： To analyses situation of drugs used of elder inpatients in psychiatry department to improve salty of drugs use. Methods： Retrospectively analyse the clinical material of 112 elder inpatients in psychiatry department of Qingduo mental health center from April 2015 to July 2016.To survey situation of drugs used of these patients to analyses drugs categories,dose,situaition of drug combination and situation other drug usage. Results： Among the 112 patients, antipsychotic drug were used mainly included olanzapine, qnetiapine, aripiprazole, fluoxetine, etc.Among these drugs, usage rate of olanzapine and quetiapine are highest,they respectively are 42.9% and 35.7%.Among these inpatients,there are 89 patients use simple drug,there are 23 patients use 2 drugs,nobody use 3 drugs.In other drugs, glibenelamide and mefformin have highest usage rate, they respectively are 40.2% and 32.1%.Usage rate of Xinkeshu tablet and captopril are higher, they respectively are 23.2% and 42.9%.Conclusion： Olanzapine, quetiapine and aripiprazole are 3 kinds of antipsychotic drug be used widely,they have good efficacy,high salty and good patient tolerance.Because of psychological depression,elderly patients have low adaptability to drugs, use slmple drug is safer for them.h should keeping the patient who use antihypertensive drugs or hypoglycemic agents under close observation for aviod occurrence of adverse reaction.
