弥漫大B细胞淋巴瘤中BCL-2等蛋白表达与临床预后的相关性研究

摘　　要：目的探讨弥漫大B细胞淋巴瘤(DLBCL)中BCL-2、BCL-6、C-myc、FOXP1、CD44V6蛋白表达特点及其与临床预后的关系。方法应用免疫组织化学法技术及荧光原位杂交(FISH)技术,对59例确诊为DLBCL样本进行检测,了解其表达情况,同时探讨与临床预后的相关性。结果发生BCL-2基因易位2例,BCL-6基因重排15例;BCL-2蛋白表达40例(67.8%),BCL-6蛋白表达36例(61%),C-myc蛋白表达44例(74.6%),FOXP1蛋白表达26例(44.1%),CD44V6蛋白表达21例(35.6%)。单因素、多因素相关分析显示,BCL-6基因易位,BCL-2、BCL-6、C-myc、FOXP1、CD44V6蛋白表达均对患者的总生存期(OS)有显著影响,是OS独立的预测因素。结论 BCL-2蛋白及基因表达提示预后不良;BCL-2、C-myc蛋白共同表达者,预后更差。BCL-6蛋白及基因表达提示预后良好。联合应用生物标记物能在一定程度上反映DLBCL肿瘤的异质性及预后。Objective To investigate the expression of BCL - 2, BCL - 6, C - myc, FOXP1, CIM4V6 in diffuse large B cell lym- phoma and research the relationship of these protein＇ s expression with clinical prognosis. Methods DLBCL samples were detected by immunohistochemistry （IHC） and fluorescence in situ hybridization （FISH） technique. Results BCL -2 gene translocation occurred in 2 cases; BCL -6 gene rearrangement in 15 cases. BCL -2 protein expression in 40 cases （67.8%） ,BCL -6 protein expression in 36 cases （61% ）, C - myc protein expression in 44 cases （74.6%）, FOXP1 protein expression in 26 cases （44.1% ）, CD44v6 protein ex- pression in 21 cases （35.6%）. Univariate correlation analysis showed that BCL- 2 gene translocation, BCL- 6, C -myc, FOXP1, CD44V6 protein expression were closely correlated with the overall survival of patients （ survival overall, OS）, they were independent pre- dictors of OS. Conclusion BCL - 2 protein and gene expression suggest poor prognosis ; BCL - 2, C - myc protein co - expression, prog- nosis is even worse. The expression of C - myc, FOXP1 and CD44v6 are all suggested that the prognosis of the patients are poor.
