手术切除与TACE治疗单纯性外生型肝癌破裂出血疗效分析

摘　　要：目的探讨手术切除与TACE对单纯性外生型肝癌破裂出血的治疗效果。方法选取我院2002年1月~2015年12月收治的15例单纯性外生型肝癌破裂出血患者为研究对象,按照治疗方式分为两组,其中10例接受急诊手术切除治疗(手术组),5例采取TACE治疗(TACE组),对比分析两组患者一般资料、近期止血效果、并发症及远期生存时间。结果手术组与TACE组在一般资料、近期止血效果及并发症方面比较,差异无统计学意义(P>0.05)。手术组中位生存时间为(30.05±4.51)个月,TACE组为(19.60±3.76)个月,两者比较差异具有统计学意义(P<0.05)。结论单纯性外生型肝癌破裂出血起病急,采用急诊手术切除能达到控制出血与根治的双重目的,并可有效延长远期生存时间,应作为治疗首选。对于无法手术切除者,采用TACE治疗也可达到显著止血效果。Objective To investigate the effect of surgical resection and TACE in the treatment of rupture extrahepatic growing hepatocellular carcinoma with hemorrhage. Methods 15 cases of rupture extrahepatic growing hepatocellular carcinoma with hemorrhage were addressed in our department,from Jan 2002 to Dec 2015,10 cases received surgical resection（operation group）,and 5 cases received TACE（TACE group）. The general condition, effect of hemostasis,complication and long-term survival time of both groups were retrospectively analyzed. ResultsThere was no significant difference between the operation group and the TACE group in general condition, effect of hemostasis and complication. The median survival time in the operation group was（30.05±4.51） months,in the TACE group was（19.60±3.76）months. There was significant difference between the two groups（P〈0.05）. ConclusionRupture extrahepatic growing hepatocellular carcinoma with hemorrhage is urgent. Surgical resection can achieve controlling bleeding and radical resection and prolong the long-term survival time,so surgical resection should be the first choice for treatment. For those who can＇t undergo surgery,TACE can also control bleeding.
