摘  要:目的:探讨CT检查在鼻腔鼻窦肿瘤性病变中定性、定位的诊断价值。方法:回顾性分析行CT平扫加增强扫描检查且经手术及病理证实的56例鼻腔鼻窦肿瘤性病变,对其组织分型、发生部位、骨质破坏情况及强化程度进行统计分析,以观察其CT影像学表现特点。结果:恶性肿瘤41例(73.2%),起源于上颌窦18例,鼻腔16例,筛窦3例,蝶窦与额窦各2例;其中有明显骨质吸收破坏31例(75.6%)。良性肿瘤15例(26.8%),起源于鼻腔10例,上颌窦3例,鼻咽部1例,颞下窝1例;明显骨质吸收破坏3例(20.0%)。良、恶性肿瘤在有无骨质吸收破坏上具有显著性差异(P<0.001),CT对鼻腔鼻窦肿瘤的良恶性有较高的诊断准确率(87.5%)。结论:CT扫描可以较准确地判断肿瘤的良、恶性,明确肿瘤骨质破坏情况、侵犯范围及周围组织受侵情况。Objective To investigate the value of CT in lesions nature and position in diagnosing paranasal sinus tumor. Method A retrospective analysis of CT features of 56 cases of paranasal sinus tumor lesions confirmed by surgery and pathology results. Organization,position,bone destruction and tumor reinforcement levels after enhancement were observed. Results 41 cases( 73. 2%) were malignant tumor,with 18 cases originated in the maxillary sinus,16 cases in nasal cavity,3 cases in ethmoid sinus,2 cases in frontal sinus and 2 cases in sphenoid sinus,which of 31 cases( 75. 6%) with bone damage. There were 15 cases( 26. 8%) of benign tumor,which 10 cases originated in the nasal cavity,3 cases in maxillary sinus,1case in nasopharynx and 1 case in pterygopalatine fossa,which of 3 cases( 20. 0%) had bone damage. Benign and malignant tumors in the presence of bone destruction with significant difference( P 〈 0. 001). CT had a high diagnostic accuracy in the nature of sinus tumors( 87. 5%). Conclusion CT scanning can accurately determine benign and malignant tumors,clear tumor bone damage and the scope and organization invaded.
