达芬奇机器人手术治疗前列腺癌合并冠心病患者的初步体会

摘　　要：目的:探索达芬奇机器人治疗前列腺癌合并冠心病患者的安全性和有效性。方法:回顾性分析我院2012年7月~2017年2月采用达芬奇机器人辅助腹腔镜手术治疗前列腺癌伴发冠心病患者的临床资料。结果:我院应用达芬奇机器人手术治疗前列腺癌伴发冠心病的患者共31例,平均年龄68(58~78)岁。术前总前列腺特异性抗原(tPSA)为17.0(1.81~66.0)ng/ml;手术全部成功,术前抗凝药物停服时间为8.8(6~21)d;手术时机器人外科手术系统准备时间为21(13~40)min;平均手术时间139(90~230)min,平均出血量161(20~450)ml。所有患者的手术过程顺利,无中转开放手术、无手术并发症。住院时间为12(7~25)d,导尿管保留时间为15(7~23)d。术后6周复查PSA为0.07(0~0.5)ng/ml。结论:机器人手术在伴发冠心病的前列腺癌患者中的应用是安全有效的。Objective：To explore the safety and efficiency of Da Vinci robot treating prostate cancer patients with coronary artery disease.Method：We retrospectively analyzed the perioperative data of prostate cancer patients with coronary artery disease from Jul.2012 to Feb.2017 in our department.Result：Thirty-one patients with prostate cancer and coronary artery disease were included in the study.Their average age was 68（range,58-78）years old.Total PSA before surgery was 17.0（range,1.81-66.0）ng/ml.All of the surgeries were successful.The withdrawal period of anticoagulant drug before surgery was 8.8（range,6-21）days.The set-up time of Da Vinci robot system was 21（range,13-40）min.The average operation time was 139（range,90-230）min.The amount of blood loss during surgery was 161（range,20-450）ml.No surgery converted to open.No postoperative complication was observed.The mean hospital stay was 12（range,7-25）days.The mean catheter retention duration was 15（range.7-23）days.The mean PSA level six weeks after surgery was 0.07（range,0-0.5）ng/ml.Conclusion：The Da Vinci robot system is effect and safe in the patients with prostate cancer and coronary artery disease.
