老年胆总管结石患者EST术后远期并发症危险因素分析

摘　　要：目的探讨分析老年胆总管结石患者内窥镜下十二指肠乳头括约肌切开术后远期并发症的危险因素。方法将我院于2012年3月至2016年5月收治的127例接受EST术的老年胆总管结石患者,根据是否发生远期并发症,分为并发症组(24例)和非并发症组(103例)。采用单因素分析和多因素Logistic回归分析对老年胆总管结石患者EST术后远期并发症的危险因素进行回顾性分析。结果多因素分析结果显示年龄(β=1.828,OR=6.221)、十二指肠乳头旁憩室(β=1.792,OR=6.001)、胆总管的内径大小(β=1.483,OR=4.406)、胆总管结石大小(β=1.507,OR=4.513)是老年胆总管结石患者EST术后远期并发症发生的独立危险性因素。结论年龄、十二指肠乳头旁憩室、胆总管的内径大小、胆总管结石大小是老年胆总管结石患者EST术后远期并发症发生的独立危险性因素。Objective To study the related risk factors of late complications in elder patients with choledocholithiasis after EST.Methods According to the presence of late complication after EST,127 elder patients with choledocholithiasis who accepted EST in our hospital from March,2010-May,2012 were divided into two the complication group（24 cases）and non-complication group（103 cases）.The clinical data of the two groups was examined by univariate analysis and logistic regression procedures,then verified the related risk factors of late complications EST.Results Logistic analysis showed that age（β=1.828OR=6.221）,periampullary diverticulum（β=1.792 OR=6.001）,diameter of the bile duct（β=1.483 OR=4.406）,and diameter of bile duct stones（β=1.507 OR=4.513）were independent risk factors of late complications in elder patients with choledocholithiasis after EST.Conclusion Age,periampullary diverticulum,diameter of the bile duct,and diameter of bile duct stones were independent risk factors of late complications in elder patients with choledocholithiasis after EST.
