摘  要:目的:评估吞咽功能训练结合心理干预在改善脑卒中患者吞咽功能障碍中的应用价值。方法:在取得患者知情同意前提下,采取回顾性横断面研究方法,将90例在脑卒中吞咽困难患者随机分为治疗组和对照组,每组各45例,对照组给予脑卒中常规治疗与护理,治疗组则在对照组基础上,予吞咽功能训练和心理干预护理,2组治疗疗程均为21d,以观察2组患者吞咽困难、心理状态(SDS和SAS评分)的改善情况。结果:对于吞咽困难程度的改善,2组治疗后有效率相比,治疗组明显高于对照组,其差异具有统计学意义(P<0.05);2组患者治疗后SDS和SAS评分均较治疗前明显降低,其差异具有统计学意义(P<0.05),且与对照组患者治疗后相比,治疗组患者治疗后SDS和SAS评分下降更显著,其差异亦具有统计学意义(P<0.05)。结论:吞咽训练结合心理干预能明显改善脑卒中患者的吞咽功能,提高生活质量,是改善脑卒中患者吞咽困难的一种有效且可靠的方法。Objective: Objective to evaluate the value of swallowing function training combined with psychological intervention in the improvement of dysphagia in stroke patients.Method: In patients with informed consent, take retrospective cross-sectional study, 90 cases of stroke patients with dysphagia were randomly divided into treatment group and control group, 45 cases in each group, the control group was given routine treatment of stroke and nursing, the treatment group in the control group based on the given swallowing function training and psychological interven- tion nursing, two groups of treatment was 21 days, the two groups were observed in patients with dysphagia, mental state ( SDS score and SAS score ) to improve the situation.Results: For dysphagia improved after treatment in the two groups, the total effective rate in treatment group was significantly higher than the control group, the difference was statisti- cally significant ( P 〈 0.05 ) ; the two groups of patients after treatment of SDS and SAS scores were significantly lower than that before treatment, the difference was statistically significant ( P 〈 0.05 ), and compared with the patients in the control group after treatment, the treatment group after treatment in patients with SDS and SAS scores were decreased significantly, the difference was also statistically significant ( P 〈 0.05 ) .Conclusion: Swallowing training combined with psychological intervention can significantly improve the swallowing function and improve the quality of life in patients with stroke, and is an effective and reliable method for improving dysphagia in stroke patients.
